  8. im not sure i quite understand..... TCA OD produces a wide QRS tachy that you slow down by blockading the sodium channels further in orde to dampen the effects of the adrenergic stimulation? So you end up with a wide QRS rhythm thats not so tachy?
  9. I thought the TCA overdose caused excessive catecholamine buildup in the synapses because of its blockade of the amine pump and the arrhythmias start from the excessive adrenergic stimulation. The lignocaine is used to slow down and reduce the number of action potentials generated by blocking the sodium channels therfore stabilising the membrane, not by acting directly on the catecholamine levels in the synapse nor the adrenergic receptors???????????? Something about what i just wrote doesnot seem to be right, methinks i will have to do some reading.......... :?

Ye Gadz! it took a while to read all this, and im sorry i didn't pay more attention, but as my young friend timmy has some people confused about EMS in Australia 000 services are covered by State government funded services. In these services and broadly speaking there are roughly 2 levels of provider. Paramedic which i guess you could say is an EMT - I or there abouts, and some form of Intensive Care Paramedic. there are no private companies or fire departments doing 000, except some first response but that is limited to some select fire departmnents and is only for MVA where they are first on scene, and cardiac arrest due to shorter response times. In areas where fire is doing this the standrds are set by the states service or government and there are clear boundaries as to what the fire department can and cant do EMS wise Now EMT as aqualification varies quite a bit from place to place, but in general terms it is developed for industrial organisations and is sold as a course by registered training companies and in most cases the name "EMT" is just that - a name. Organisations are not required to have this level of training in their workforce, but there are advantages in insurance etc. It far exceeds what is the minimum requirements occupational health and safety wise. A few of the EMT type courses i know of are trained to use the drugs found in remote areas. EMS in remote areas is generally covered by Royal Flying Doctors Service (RFDS). Remote locations also have a kit that contains some of drugs and interventions that can be performed by a trained first responder of some sort, and with the permission of the Doctor who is on the plane flying to their location. For example anti venom can be administered under medical control to a patient at a remote location. EMT type courses in aus go a little bit further into this to allow some advanced airway management procedures and fluid resus, but is usually under the control of a doctor from RFDS, or an onsite doctor employed by the organisation - its placing a higher level of first responder at locations that do not require or cannot afford a frontline ambulance paramedic The other place you see EMT type people is at motor racing, sports etc Who again are trained to a level a bit higher than advanced first aid, but again below ambulance paramedic So yeah some can do tubes and IV's, none do 000, and EMT is just a broadly used name. Its a shame, i work for a company who does event support as en EMT type person, and we get chided very harshly by a lot of the state service guys from being wannabe ambo's, and trying to take their jobs - yet they will also whinge when its them at the football handing out icepacks instead of us - really bad attitudes toward anyone other than themsleves Dusty, as to officers taking ambulances home - usually done by people who are rostered on call. In the case that the ambulances in station are all busy and a job goes down, they page the on call and 2 cars with one officer in each go directly to the job and either await a car to come and take over, or do the Tx and Rx themselves P.S. 000 = Aussie 911

  12. got nothing to do weather or not thunderchild has used the ZAP Best evidence says that the ZAP has a lower 30 day survival rate than CPR. Literature is as slanted as any other opinion, BUT it holds far more value in levels of evidence than expert opinion Thunderchilds decision for CPR is more than valid and we must remember that pre-hospital success is far from patient viability. If its true that the 30 day survival is lower, than it should not matter that it works well for us, it does not work well for them, and thats the bottom line

  17. Hey dont laugh man! I helped a vet do CPR ona horse once, while i did compressions (literally laying the knee in) his assisstant did mouth to nose and he shaved this horse with an electric shaver, pulled out the gel and shocked the crap outta it with a lifepak 10!!!! Yes it lived... Ahhh i defy anyone to show me "Field Surgery" like that - apparently horses, thunderstorms and fences in the dark along with arterial bleeds make horsie a vewy sick wittle pony.....................

Then, im gonna grab the Oxyviva 3 that is like 25 years old, weighs a ton and made of stainless steel and check it over Then i will check those 2 Lifepak 5's, and the half dozen or so Lifepak 300's we have All of this stored of course in a few 1988/89 F250 with the big V8 that hauls ass, the onboard suction runs of the manifold pressure and it still has the jordan frame racks on the cieling When i figure out the image hosting thing ill throw some pics up. I swear i was born in the wrong decade - i wait for the day i can get my hands on a couple of plano 747 box's to put my fishing and modell making gear in, lifepak 5's and 10's as far as im concerned are sweet and i wanna rebuild a 42' model willyz jeep, i think that the old 6x6 studebaker trucks are a good drive (the one i was in had "US Army Lend Lease" written under the bonnet) WOW! im living in the twilight zone!

Well, in an effort to buy a small "jump bag", i ended up with a stat packs load'n'go for 80 bucks AU (instead of the 160US its worth - thanks EBAY!) and the thing is just massive - far bigger than i thought it would be, so i have done the only thing i can do, and that is crammed if with as much stuff as i could. Bear in mind that this thing gets used every weekend, and i dont have to pay for its contents, and when it does gets used it quite common to have to walk 5 or so kms into the scrub and sit there all day, so you have to have a lot of basic dressing with you 8 9x9 combines 6 Triangular bandages 8 7.5 bandages 4 5cm bandages 2 icepaks 12 Gauzeswabs 2 space blankets BVM - had a pocket mask but after a couple of hypoventialting head innjuries and only 1 oxygen unit to 2 patients, i turfed the pocket mask set of OPA's 8 30ml saline 4 30ml chlorhex Panadol Mylanta Aspirin 1 stick of methoxyflurane varies antiseptic/iodine/alcohol swabs about 20 in all 10 10x10 non adherent 4 eye pads 2 20x10 non adherent 1 30x20 combine B/P cuff Steth Thermometer Penlight Tweezers Trauma Shears Minimag 2 glow sticks 2 micropore 2 elastoplast assorted bandaids - 50 in all (thats how it came in the box, so i just chucked it in) Sharps container (splinter probes) Gloves Hypafix damn sand based turf) Glasses 6 or so of those bandage pad combine things 2 forearm cardboard splints - cost nothing and taped together you can do an entire limb I dont bother with collars cause we cant use them for first aid yet - though we will soon, and besides, at 150 bucks a set they can supply them and then ill use them Sad thing is that the bag is still half flaming empty and im trying to figue what else is actually useuful and slightly more essential to put in there - for sake of stupidity im considering a tin mug, hexamine stove and coffee Im such a loser....... Having said that, this bag is going to go hard when i go camping with it next week
