  4. i love the fact then when we start bitching about "delaying transport" to a trauma centre that there is only ever an inference on the IV. For example, you have never seen anyone say "You delayed transport of major trauma patient to put a c-collar on?!!" or a pelvic splint or provide pain relief for their extruciating injuries. Where is the attention to wholistic patient management?? Where is the attention to time management of the whole case from start to finish? Why do we instatly go "you fucked up" instead of "Good work guys, that was a tough job", and the criticism is only ever based a what amounts to a few seconds in time over the sometimes hours of a single case.   13. Without invasive ICP monitoring it not possible to really control or monitor you interventions. The effect of you vasopressor for instance could either be very good or very bad, but you will never know without a transducer in place. If you grab the algorithim you have and start playing with the digits (MAP and ICP) youll find that a B/P of 120 will maintain resonable CPP over the broadest range IF the ICP is not too high. Elevated systolic pressure is not necessarily good but it will maintain a reasonoable range of CPP. Moderate to severe hypotension, even in a patient with a ICP at the high end of normal is nearly always inadequate. So, mainating what you can. Keep the ETCo2 stable at 35mmHg ( I think the magic number escapes me) and treat hypotension. if they are coning there is nothing you can do except influence the time to difinitive care. Of course, i could have just made that crap up, but it sounds good
  14. Sorry man, im not sure what your asking. If its about GTN my answer is no. The pt is denying "chest pain" pr se (even with the shoulder tingling) but the inferior on an already unstable patient even with a B/P of 160 would be a no go zone to me... and thats a pretty big statesment as going by the book (our book) he is indicated for it, so i'd withold and keep frank-starling happy. Also, at 50 y/o i dont think he is likely to have well devloped significant collateral circulation, so there is a chance the GTN will have minimal effect with a whole lot of risk attached. Treating with aspirin and diesel is more than appropriate, he either gets drano from the local hospital or a chopper ride to a PCI lab.

I left indonesia and egypt off the list by accident You'd be surprised to know that for some australians erved with the british in russia and im pretty sure canada sent a whole expedition there for the civil war, so for some the first world war for them finished in 1919. You wont find a record of that in australia because the government didn't want its name on it, so the few we sent were rolled in with the british, even though they were to still wear australian uniform's. In fact, Australians won 2 victoria crosses in that theatre of war. Some war memorials in australia for the first world war list the "great war" from 1914 to 1919 because of this There were also a bunch of guys still in the middle east who either voluntered for, or were pressed into service in egypt to suppress some kind of anti-british uprising. A few guys were killed there after WW1 finished but before they could ship home, hene why the inclusinve dates for WW1 serviceman runs all the way to 1921 for the service records... bloody empire building.
