Ok get one of the firemen (or cop) to hold c-spine and do a rapid head to toe what are my findings, partner hooks up o2 on nonrebreather 15 lpm and gets baseline vitals,what the situation look like can he be long boarded right there or any other problems with spinal immobilization. In the head to toe also looking for amulets, wallet for any history that might be important. whats his breath smell like sweet, alcohol anything oh yeah way back there is the airway patent what do we see in his mouth.
  4. Your right (man that felt weird) I think the only way to change that is threw a coordinated effort to promote EMS and change its image. Know I acknowledge that my experience is in a small rural area but what I have heard over and over again is the same here they (the public) are willing to pay for what they see and use every day so they will harass the politicians into getting them what they want. People have the inaccurate concept that EMS is supported by trip charges and has no need for extra funds. Know fire has seen the light in many respects and learned to promote themselves. Does anyone realize how big of a return is generated in goodwill towards the fire department for doing community work such as passing the boot for Jerry's kids. And if we have a big fire we do our best to get in front of the TV camera. A chief told me yrs ago you don't have to be a good firefighter if you have a good PR man and sadly its true. At a auto accident or medical call we don't talk to the media cause they ask stupid questions about the patient that we can't answer but if we can charm the media then when funding rolls around we have a better shot at winning in the political arena.

  7. What do they think of us. You pretty well hit it right on the head. Till there up to there ass in alligators some see us the bastard child (we are a all risk agency). But there is a bright side we have a auxiliary that raises 30% of our revenue. We sponsor the local food bank at battalion one, we are the polling place for our area. We have a community hall and we are a emergency shelter. We do a lot of safety and prevention with our schools and provide mutual aid to state, federal and local agency's. So many of our small community take alot of pride in there Fire/EMS Department. Now for the hard part getting them to volunteer. Wow drop that one in a room and you will be standing alone like you ripped some nasty gas. I fear that the days of volunteerism is on the wane with no improvement in site. Will they support a paid or partially paid agency in time I think so. I have a vision of our ambulance going to a no charge status with the funds coming from tax roles. And believe it or not I wasn't stoned when I came up with that one. People moving here are coming from areas of the country were if you pick up the phone and dial 911 help is there in five minutes or less. I think they will be willing to pay for that her in time. The hard part is Montana's have authority issues and it takes a bit to change there minds. With our folks if you ask them for a donation they will reach in there pocket very deeply but ask to raise taxes and you better be ready to fight your way to the door cause they'll will lynch you.

  17. watched the dash cam vid and have watched the other as well as read the statements. I can see with hind site some things that could have been done better by the medic but I can't see anything he did wrong really, yes abandonment could be stated in this incident but I'm sure if he had none how it was going to play out he would have stayed in back. I have to wonder still if there isn't some behind the scenes bad blood here with the way the cop reacted. The cop to say the least was unprofessional in his conduct, and to do this knowing he is being recorded makes me wonder if he is suited for law enforcement work.
  18. OK AST is elevated still haven't pinned it down Muscular Distrohpy could be a player but the abdominal pain doesn't fit. Still a work in progress but given that if he still remains stable set him up for that 72 hr transport and monitor him. Oh yeah that LFP was a liver function panel my bad but I think we got there.

  20. Hybrids have there own saftey hazards to watchout for, life altering voltage, acid spills, hydrogen storage on some and most have all the new air bags. There are a lot of info sites out there but Holmatro has some good info on there's. Don't know of any sites with info geared toward EMS. But if your going to crawl around one before the paitent is extricated then you should be aware of the hazards.
  21. The point about the article in question is not a look into fire based or nor but rather into city management. The equipment is by no means the big expense in any emergency services because they may need replacing but not right away and all at once but wages and compensation is comprising the lions share of the budget. Even at MCD rates its still there largest outlay and we hope the would pay more for EMS
  22. Err sorry you didn't frame your answer as a question. The problem is not volunteers its bean counters. There advancement is based on getting the most for less. And when you consider Wages are the highest part(up to 70%) of the budget they will cut staffing with a broad sword. Look into the financial problems of Kalispell Fire dept. it is in the local rag (Dailyinterlake.com) The city council is willing to reduce service and rely on the neighboring volunteers. And this is after they screwed up the budget. It is a good read into city budgeting without regards to your beliefs in fire based EMS. People are uninformed and honestly could give a crap less. They only remember your out there when they are in trouble then they want you there in 5 minutes or less. And honestly they don't care if your paid or volunteer. Isn't apathy special. If you want to change the system it's image that is lacking not volunteers creating mayhem. Why is it people always wave at a firetruck but probably don't see an ambulance unless the lights are on.
  23. Yeah it disappeared weird anyway looking into several things wanting to look at liver enzymes, white cell count ect. Spoke to a nurse that runs with use he recommended this blood work rather then being to narrow sighted in what I'm looking he recommended these because they would look at a full metabolic panel. I will get the abbreviations expanded next time i talk to him. The urine tox screen is to look into contamination from anything from home brew to heavy metals( i think I get that one to from this test) I am leaning towards a liver problem but have not left the hernia alone problem is if its a hernia and no imaging equipment I am not sure where to go with it. If it is a hernia and we hit the jackpot on the white cell count its time to boogie. Oh yeah I may have pooched the LFP for LP or it could be local differences I'll check.

If they cut the volunteer organizations budgets then it is logical that they would cut paid agencies even more considering wages could account for more then 80% of most emergencey services budgets. And after they cut the wages what will they augment the staffing shortfall with why vollunteers of course
