  1. I am amazed at the naive illusions some paid EMS "professionals" still have about volunteer services. Do they not realize that there would be paid services in all areas if the people could afford them. Sadly that is not the case so people rely on volunteers to do what is needed and have for many years in fact most if not all fire and EMS services started as volunteer. As far as that hapless soul working at McDonalds perhaps he or she has reached the top of there skill levels and peeked out. Perhaps we should remember who were there for

  4. 6 yr old shot in the abdomen with a 22. She looked up at us and asked 'am I gonna die'. I looked her in the eye and said no not today. We treated we transported she flew to Seattle, hit her pancreas and VC. Almost lost her several times I guess she had a lot of internal damage but she has made a full recovery saw her 5-6 months later. Looked good and she was taking hunters safety with her brother to desensitize her to firearms, she was obviously traumatized by the experience. Followed that case for weeks and kept in touch with the family to she how she was reran that call a 100 times in my head. Kid that did the shooting is in the JV system know hopefully they can straighten him out he was only 10 at the time. I did learn from that experience not to beat myself over the head on things I could have done beater but to do everything I can to improve my performance next time.
  5. Unless your LZ is close to the incident you will need to transport to it. Also the IC needs to coordinate with the local ER's that will be receiving and a place will be needed to care for and monitor the Greens on the bus if you take them to these small hospitals your describing they will be no better then if they stayed were they were. The hospitals will be over burdened with the kids you launched. I think I would send the most urgent patients by air to lessen the load on the local Hosp. and let them treat the greens. While transporting the greens a one on one history of each is warranted some may not have ingested anything bad at all. I think you said 5 helicopters how quickly can the round trip to make my plan work. You can assume with a emotional trauma like this something is going to show up medically in the greens.
  6. These type of calls are tough because they are a contradiction to our job description. But it is somewhat easier after you realize that most of these victims are suffering from depression, a mental illness. I am sure that many will argue that point but it holds true in most cases. This being the case I think they need to be dealt with the same as any other patient (with obvious exceptions). It is still frustrating to get a call like this and I often get the desire to slap them along side the head and tell them not to be a dumb ass, But if I cant do that to DUI's I will refrain on the rest as well.
  7. Ok so I have enough transports but not enough ground crew what about mutual aid? Then lets start using cops they can carry litters and do traffic control. Your shortage on resources is not uncommon. Lets not forget to use other local talent is there a local game warden or forestry division, search and rescue all of these agencies should be trained in ICS. Lets not forget that your helicopters coming in could be heavy one EMT or nurse even a doctor if requested. As far as Fire or EMS personnel being trained in MC if they are trained properly in ICS it is not a difficult job do fit them into the incident. And yes they should be trained in MC.

  9. Ok start with at least ten transport ambulances the bus and 4 fire department engines. Set up a command post with the following over head IC, Operations Chief, Triage Group sup. Transport Group sup. Traffic Control Strike Team Leader. and a Public Relations Officer. Place some EMT"S in Triage group probably 5 and use firemen to help load ambus. Use FF's to set up a LZ or two. Put a FF and a EMT in charge of site analysis for contaminants make them a group if you like but i would merge them with triage so you dont exceed your span of control. After that its simply a matter of working threw the process.

  11. fishy has me stumped but I think she injested something caustic and it is irritating her digestive system or she is bleeding internally also question what she took get a als rendezvous and treat for shock. If ingested substance meets protocol give activated charcoal. lets do a head to toe for injury sites or injection sites. and I would be ready to do a fluid challenge if she keeps losing BP ABC protect airway and get on down the road
  12. We will often use light and siren in route to the call but usually downgrade enroute or on transport to the hospital. Our purpose is not to drive fast with our heads hanging out the window going woo woo it is to be seen and heard for safety. Once we reach the wreck, sick call or what have you lights are used to mark the scene and be visible in fowl weather or on roads. This is for safety as well as helping other units locate us. From a legal stand point it is inadvisable to run code with a BLS patient if they start circling the drain you can upgrade. Also their is no code 2 and 1/2 its lights and sirens. That being said I must admit we turn our siren off allot who's gonna hear us a deer? Another thing I have found is you roll up on someone and hit the siren you get instant brakes, is that what you really wanted. If your gonna light some one up do it before your tailgating. I must add that we have a very minimal traffic problem and people in areas that do would probably have a different point of view.

  17. I think this is a psych issue. Bipolar just went threw a traumatic experience looks up to us yucks and freaks. Blood sugar could be up due to pysch meds and unknown med history but I have dealt with bipolar people before and its a wild ride and unfortunately there is no exact science to the meds they take it is sadly trial and error and this could be error. One thing i have a question about is did the patient respond better or worse to a opposite gender person then themselves or same gender its weird but some times bipolar people react differently and you have to change up who is dealing with them. Also tho the numbers dont indicate it yet they are heading towards a stroke consideration and I think some pysch meds have a problem with that.

  19. Cars rarely explode if they do catch on fire the fire typically starts in the engine compartment. MVA caused fires can result from a wide variety of causes but the most dynamic ones are from ruptured fuel tanks. The physics of getting a explosion require numerous safety systems to fail and a lot of bad luck all on the same day. That being said there are some very dangerous components on a vehicle involved in a accident. Airbags for starters are a real gasser they not always or only deploy on impact but some models can deploy afterwards ( it is meant to protect against roll over injuries. If these devices have not actuated they stand poised to release at a unknown time on the first responders involved in the incident. And contrary to popular belief they are not deactivated with battery separation from the vehicle. With side curtain, driver and passenger airbags to contend with it is best to become apprised of vehicle designs and technology. A good safety margin to stay away from air bags is as follows: side curtains 8", driver 14", passenger 20". So how the hell do you work with those requirements (very carefully). Manufactures need to be made accountable for the safety of first responders in their equipment. Know the new Hybrid vehicles carry their own hazards including high voltage, large battery's full of acid. And lets not forget all the compressed charges used on a vehicle including bumpers, seat belt pretensioners, hatchback supports and the like. Fire and damage can make all of these devices fail in some exciting and dangerous ways. Another saftey concern on a MVA is if they do catch on fire the toxic levels of the gases released. You can assume that you loss 3 years of your life span with each breath you take of those fumes. Hopefully this will reduce the complacencey we tend to get into on MVA's tech is changing and we need to stay current with it.

In MT I see jobs for EMT-B's now and then some private ambu's Commercial operations hire to for plant safety and the State Fire agencies hire B's for wildland season on the bigger fires which is a cake job most of your clientele are 18-40 with no med history so most everything is straight forward. its mostly seasonal but if you get on the teams its a sweet gig and you get to see lots of country.
