  3. Sorry to hear this. I have not known Robert personally and I`m not long enough around here, to have read a lot of his posts, but reading those posts you guys wrote in this thread, gives just an idea of what a great man he must has been. Rest in peace!

Could you guys post an example of your (or any US-based) form of a PCR - I don`t mean what you actually write in it, just the layout of your protocol (only if you are allowed to oviously), I´d be really interested. A (really great and nice, not that it would matter, but still ) Doc told me once: "You`ve only found and did what you`ve put in writing!" In my opinion, propably the best way to sum up this topic.

  6. I guess he`s an epileptic. What medication does he get and how many fits has he regularly in a week/month. If there`s a major discrepancy (which`ll propably be the case), I`m taking him in. Did the mother already give him any anticonvulsive medication? Also: is there anything different about the seizures or his general behaviour? Did the mother notice anything (that would point into the direction of a neurological incident beside the known epilepsy) I`d take a round of vitals, keep the oxygen on him, give him an i.v., start with 10 mg Diazepam. If he stops fitting - get him in the truck and let`s go.

  9. I worked at a company for a couple of months, where you were only allowed to watch TV in the night shifts. Know of a couple of other stations around here that have this policy. At my station - TV most likely runs 24/7 as long as there`s someone around.

  11. You didn`t come across as if you were trashing those who carry gear, dude. I just wanted to make clear that I`m neither doing that. As for the vests. Since a few years, we have to carry them in cars permanently, before that, only truck drivers and cars used for business of some sort needed to have one in it . We get vests from work, though since we have our normal jackets with the reflection stripes and extra jackets in the car (mainly used for rainy weather since they are thicker), I`ve never used the small one. It`s still shrink-wrapped.

    Has to be due to your numbers of users. Around here, you wouldn`t just push an antidote without significant suspicion of the use of the drug. Is flumazenil also in your SOPs for altered mental/conscious status?

    Honestly, it`s not such a big deal, it`s just liquor with a very high percentage of ETOH (around 70, depending on the brand). Since the Thujone content is governed, it isn`t more hallucinatory than any other ETOH. We sometimes drank that stuff, there are different "absinthe rituals" to consume it (mostly it`s 2 parts water, 1 part absinthe, then put some sugar in it and lit it to fire). You just have to be careful not to drink too much, out of obvious reasons. I`m not sure how the wormwood is manufactured in absinth, but I guess simply adding it to a drink won`t set free the thujone (or does it, can`t imagine it that way, gotta research that). Can`t really see why it couldn`t have been only ETOH. Sounds like she just got herself into the excitation stage - I`ve seen patients present themselves like this after (only) consuming alcohol.

    I guess there are a lot of possibilities for chronic illnesses that could affect your ability to cope and deal with such a consume of alcohol. @ERDoc: Is there a particular reason for Absinth to be banned in the US (well got to be ) - does it maybe has to do something with the Thujon issue?
  15. Same here. Not much else you can do, bad luck for her, but if that`s the way it is you can`t change it. Course you could go for invasive blood pressure in the last instance, if you`re able to do it and your monitor supports that feature.

The funny side was, that just at that time, we were going through the anatomy of the heart, and we were standing there, making that (really old) joke about tawara-branch-fractures (in the first instance not to fool her around, she was just standing there coincidentally) and she was asking questions about that Fx, wondering, why she never heard about it. So we just spun a tale about it, but with real obvious sarcasm. Turned out, she took our words, though. Hm... I`m guessing this sounds really mean when you weren`t there, but it honestly was just a laugh and she wasn`t mad at us, or anything.
  17. Absolutely! I don`t want talk the people down who carry jumpbags and the like in there cars, though. Personally, I just don`t see the necessity for me, but maybe you guys are more enthusiastic than me. At work, where I have all the material, I do my thing, and the patient gets it all, but if I`m driving around and stop to give FIRST AID - I`m only giving first aid and wait for the ambo or my colleagues to arrive to do the fancy stuff. And honestly - they don`t pay me enough, for me to be willing to purchase all that equipment (would I be willing). This may get me on the wrong site with some around here - but after all, it`s just a job. And just because I can use a defi and know how to handle all this equipment, doesn`t mean I have to carry it around privately only because I may meet someone who could need that -> that`s what`s the ambos for, and that`s what I get paid for when I`m sitting on that truck. Again, I`m not critizising those of you who have the motivation to do this, just my point.

    As far as I know, Naloxon or Narcan does only antagonize opiates - I can`t really imagine opiates to be in that drink (never heard of it, though). There are of course possibilities to produce fluid opiates (i.e. laudanum), though I can`t really imagine that they use opium poppy in that drink. All the more mysterious, why your patients seem to respond to narcanti - could be coincidence though. The ingredients that gave Absinth the "kick" in previous times were for the most part absinthin and thujon. Thujon especially triggers hallucinations (can lead to epileptic fits, too). Because of that hallucination aspect, Absinth is also called The Green Fairy (referring to its green colour [at least in the past it was mainly green, there is also black and I think red absinth] and the hallucinations you could get). In Europe, the amount of Thujon in todays Absinth is confined to 35mg/kg. Don`t know wether such a regulation exists in the US. I guess you would taste wormwood though, it`s a pretty obvious and characteristic taste and I can`t imagine it mixed with fruit flavours, so I guess there is no wormwood in that drink.
  19. Lol. Buddy of mine and me told a chick in our Paramedic course how to treat a Tawara-branch-fracture! (In german "branch" is more like associated with thigh, so it sounded more realistic). She believed us to a hundred percent and some weeks after that asked our teacher in class wether he could go through the specifics of tawara-branch-fractures again! (To be honest, she was neither one of the best nor the smartest in class )
  20. Well, Bernhard, seems like you enjoy to do the work. Personally, I wouldn`t wanna work at the Oktoberfest, even if I were paid for it. Wasn`t actually there, though (lots of my friends were, though). It`s just not my thing I guess, I don`t really fancy events as big as that. Furthermore, a Maß for more than 9 Euro is just way too much. Although the prospect of naked chicks does sound good...
  21. Yeah, I can relate. Been there, too, though not by gunpoint - but you don`t really need a gun to do someone. When it cómes to pranks - often heard the story of someone getting some lidocaine in their coffee.
  22. Well I do understand the concept of cheating - though I propably wouldn`t do it (never say never). But this guy`s life seems to be wholly orientated towards timing and organising his affairs - would be wayyyyy to stressfull for me.

  24. Yeah, with the LP15, we don`t have that gimmick (not that I´d miss it much). I actually meant checking the pulse rate showed on the monitor when you`ve got your finger clip attached.
  25. I mostly need only some verses to get into the rythm right, the I`ll hopefully stay there for a while. Of course, you could also just look at the monitor, but that would be too easy, wouldn`t it.
