  3. Are the superinfections a significant issue at this time? Are they predicted to be radically significant in the future? In PNG, at least in some of the areas that I've been in, antibiotics are handed out like water. Amoxycillin is the cure all from headaches to the common cold...the healthcare providers understand, many of them at least, that this is innefective for most things, but the local populations have seen people get healthy after treatment with it, for appropriate issues, and extrapolate that to an almost magical cure for everything, and it's easier for the HCPs to simply hand it out instead of arguing with them..

You know Mike, I would be happy to collect for such a study, but man, getting them out of the country in a timely manner would maybe be tough...not sure. Maybe those doing the studies could make a deal with the cooperating countries...but PNG has many secrets that might make them leery of participating...for instance, according to the govt, there's no HIV/Aids in PNG to speak of...

  10. I thought that the original post was near perfect. What would have changed Mike if she'd said she was looking for information for the future? You'd have told her to come back in 10 years? Why does it piss you off that she's taking the time to research and prepare for a part of EMS that gives so many trouble instead of waiting until she's in the ditch to do so? She had a question, she asked it, the fact that she's not in that situation 'now' changes nothing about the information that she's looking for. Sorry your having a bad day, but sniping our new members probably isn't going to make it better. Great question girl. If you're thinking of medic instead of basic only, the advice on how difficult school is is worth it's weight in gold. If you're going to become a medic then go to school before kids, and make sure that you have support when you do... As far as the shift work, that will depend a lot on your husband. My wife is a doll..she molds to anything and we just adjust ourselves to it, but she's a full time homemaker. ..School, clinicals, work, family...without a pretty solid support system it can be to much for most of us..the job? It's just a job like any other pretty much...you'll have to make allowances, but everyone does, there's really nothing special about EMS that makes that more difficult in my opinion.. Can I ask, why EMS instead of one of the health fields that actually pays the bills and has more normal schedules?

  16. Laura Anne! It's really good to see you here! The City is less when you're gone... And I don't know that acknowledging the difference between the sexes in a positive way is sexist, but if it is, then I'm ok with that... And Chris, the evolution of our brain hasn't evolved us beyong common inherent sexual and social differences, has it? Surely you don't mean to imply that you see the whole world as a bunch of asexual, equally attractive/unatractive biological machines only interesting for their ability to provide life support for brains? I think that we've evolved to the point where we should be able to recognize that differences exist but those differences do not necessarily equal a weaknesses. And if, even if one trait does test weaker in intellectual/survival situations, that it can't be taken as a judgement of the entire being without all others being weighed as well. We've certainly evolved politically to pretend that those differences don't exist..but that should be defined as devolution instead I think, if we've begun to deny those things that are intuitively apparent, and scientifically provable.
  17. Please don't say 'prick'... I'm leaning toward a heater malfunction and CO poisoning. Any symptoms in the friends that have been caring for him? Probably pretty early at this point... Cold nights, general failure to thrive, it appears that he's shunting a bit (difficult finger stick), despite shunting he's got an SPO2 of 100%. An SPO2 of 100% in a patient breathing normally always makes me ask "why?", especially when it's cold, or near anything to do with mechanics or heavy equipment. You've mentioned a missing vital sign, do you mean skin color? Or temp maybe? If not, then I don't know. I'd have the hosemonkeys check for it if they're on scene, get everyone out of the house and interrogate the crews and friends for symptoms, otherwise I'd put him NRB/15L, prepare for transport and notify the receiving hospital of a possible CO issue. I'm willing to bet that this will be close...

  19. That's the gun that you push down really hard on until you hear the "snap" and it's supposed to be in place? I didn't like that, though I only used it once, and it was easy, though failed. I used it when one of the docs asked me to start one..I can't remember why now..I had a borderline bariatric patient without the bariatric needle. I mashed the hell out of it, but maybe my guess on what they considered bariatric was off.. I'm sure it works fine, I just didn't like the whole, "push and pray" thing...Do you know what I mean? Not choosing when to stop, but hoping that it was placed right based on something else that I don't understand..

  21. Pretty far off in the ditch as there's only one of the I/Os that's meant for the sternum, I think? I've placed more than a dozen I think, and all in the tibia...very easy to do. I've used, I don't remember the names and don't have enough speed to search for them, the electric drill (FAST?) and the hand one..yeah, can't remember... But, either way in the tibia, adult or child, it's really easy. But your landmarks are pretty important in the child so that you stay distal to the growth plates...

  23. Yeah, that probably makes a difference. And maybe 7/7 is standard there and the contracting company can just piggyback your guys with theirs? Not trying to be a know it all my friend..just tossing out ideas...where do I submit my CV? :-) Someone told me, Moebs maybe?, that they don't have the same requirements for remote work up there and hire American medics often...Just a thought...
