  2. My son is adopted, we had no problem getting the birth certificate even with the hospital bill unpaid as of the date we got the birth certificate from the state of Oklahoma. I can't imagine that a Vital record would be held back by a hospital based on bills not being paid, think of all the death certificates that would have been in limbo for litigation of that persons death and who would pay for that. But I don't know the entire country. But I think we got my son's birth certificate within a week or so of his being born and our taking him home, as a matter of fact, I think we got the certificate within 3 days of our arriving home.
  3. Hell, that's nothing, I helped Mom deliever a baby on a sofa in a run down trailer in a blinding snowstorm in the middle of the night and I didn't get my name in the paper. I feel jipped.
  4. And to do away with lights and sirens during transport means we will have to do away with LIghts and sirens all together because if you have them available people will use them.
  5. But Doc, I'm sure that you know doctors who don't really care and are just in these URgent care centers for the money right? Or were you referring to the student? We might not be talking about the same individual. I'm saying that the doctor who is running things might not be too concerned about Dwayne's bill. Makes me wonder if this isn't a pattern that is prevalent at this particular clinic? And yes in a perfect world, the doctor should be worried about it but he just might not give a shit

  7. I think that once the world ends and those of us left behind in certain pockets of the planet that was not destroyed, we have to start over. Who wants to be my 2nd in command? I already have my command post picked out. I have enough supplies scouted out that our new UTOPIA should last another hundred years or so. Plus all the cheezit's and soda that you can stomach. I've got a core group of people who can make sure all amenities are taken care of. You just have to provide for your family and contribute to the collective. And no we will not call ourselves the Borg. Everyone will have a say in how things are done.
  8. Just consider that this guy is hosed. I also had a hard time watching it. I really do not know how the trauma surgeons are going to fix it. I also did not get all the way through the video as well. I got far enough to see them sort of push the guys head back together or a facsimile thereof.

  10. Yes, that would be me, "Ambulance Driver Ruff" and I even referred to myself as an "Ambulance Driver" at many times. Hell I still do at times.

I only threw in the ammonia level due to my friends ammonia level was so high a couple of times and his was missed (nearly fatally twice at a local small town hospital) and having that ammonia level could have directed his transport to a higher level of care than little doc in the box than the level 1 facility that he should have been at to begin with. And of the Istat's none have ammonia so it's a wish list not a requirement. The drug test one is a good one but you don't have to have it in the field but we in our ER had them at our disposal but they were based on urine not blood so the patient had to either give you a sample or you had to catheterize them (we could do catheters in the field as well). But I still stand by my list other than the ammonia.
  13. none of the test listed in the link have a CBC as a result so you might not get your wish Wolfman. But it might be found from a different company, just not Abbott.

Yes I do Doc, Here is the Istat website with a comprehensive list of their tests. http://www.abbottpointofcare.com/Patient-Care-Settings/Hospital/Emergency-Department.aspx I would think that an ammonia level test would be beneficial, as well as Troponin, Others in this link would strictly be used in the ED setting and not valid for EMS work. One other would be a valid Alcohol level as well as a drugs of abuse test. I think if I had my choice I would rank them in importance 1. Trop I 2. HGB/HCT 3. Drugs of Abuse 4. Alcohol 5. Ammonia (strictly a routing tool to the appropriate facility - super high ammonia gets a non general hospital but gets a specialty hospital) 6. CMP/BMP 7. Glucose(we already have the machines to do this)

  17. So what is the Gold Standard? Is it the ETT or is it the Surgically placed airway? If I trached a patient in the field have I placed the Gold Standard even though I've never done one before? I'm terrified to trach someone so I'll probably be so reluctant to do it that I'd sooner give my left testical and right hand but of course I'd do it if left no alternative but I don't think that a paramedic placed trach is a gold standard.
  18. The Istats are nice but CLEA (I think, not sure of the acronym) would need to be involved to allow that since it is essentially a lab test. Hospital based EMS systems are best suited for this type of test and the rolling out of these things. Once these are rolled out in several systems and shown to be effective in patient care pre-hospital, then I would look for them to be a standard of care within the next couple of years. They are pretty much idiot proof if the ones that I've used would be used.

  20. Actually I just saw a officer today with about a dozen camera's mounted on his car and I followed him to a Steak and Shake and struck up a conversation with him and he confirmed your info. He says they hate em too but he said that his town has the highest stolen car recovery rates in the metro area so they couldn't resist that grant money that came with the cameras. He said that there will be a total of 12 new cars equipped with these camera systems which for this town that's I think their entire fleet. So I guess every officer will get a new car.
