  4. that's not rude, it's a fact of life. I for one will never ever ever ever fit on that silly looking thing. You come at me with that thing and I'm liable to make you show me how it fits you without being painful or screwing up your back. That is a worthless piece of equipment if you ask me.

  6. I worked with a partner who thank god trusted me enough to tell me they were thinking of it. When asked if they had a plan they said yes. I asked when they were going to carry it out and they said they already had begun it. Needless to say we stopped working and went straight to the ER and he was treated for an overdose of Dilaudid. All turned out ok and he is still working and is a better and stronger person due to the treatment. I can't imagine what he was goin thru but after a divorce, a car wreck and a ped code we didn't save the shift before he decided to end it all. But he trusted me enough to allow me to help him get help. My work was sympathetic and provided him with tremendous support thru it all.

  9. Sassy, sorry for your services loss. Please accept the entire EMT City's community condolences on this. It is always a terrible thing when one of our own get hurt and even more so when they are killed. I have known many who have been involved in crashes and some were not hurt, some hurt significantly and others killed. It is a tragedy but it's a risk we all take in and out of the ambulance. What shocked me on your post was the fact that parts of their seatbelts were found outside the wreckage. MY GOD, how bad of a crash that must have been. How fast were they goin? That is one of my main question. For pieces of the seatbelts to be outside the rig that must have been a horrific crash. You have the support of this group. Good luck in your healing.

  11. I've always been fortunate to be able stay on scene an extended time if needed. But I've always had LEO's there becuase that was the procedure. Code blue call and LEO's got called. I will stay for the family until I get called out again. I do explain that I will stay there until I'm needed elsewhere. Always worked for me.
  12. Malpractice insurance - depends on how much is paid out and how much you pay in. Remember, in a lawsuit those with deeper pockets are going to be the ones who get hit with the suit. If you have a policy for 3 million dollars and your companies policy only covers you for 1/2 mil then who are they gonna go after. Second - car insurance - do not get extra insurance for yourself in response to you driving the rig. Your employer has all the insurance you need. Again deeper pockets. If you get in a wreck in the ambulance or rig, and your company only pays out this much then you might be responsible for the other. Plus and a huge plus too you get in a wreck and your insurance company has to pay out any amount and you will be dropped like a hot potato and you will find it damn near impossible to get insurance for your car again.   13. Now there's a name I remember well. He taught my medic refresher course. Some of his scenarios are legendary. He's a true gem among instructors. Unfortunately some have said he is very biased against ems'rs who do not work for his service. He's one of the best instructors I've ever had the pleasure of taking classes from. He's forgotten more about EMS than most people know.
  14. You have to let the family know what you are doing. Tell them you are breathing for their loved one who is not doing it on their own and you are providing the pulse to the pt. who is not doing it on their own. Tell them that if you stop they will not start up or something like that. Most families know when it's time IF the family throws a huge fit about not transporting then by all means transport. Sometimes families take the news of the death better in an ER than at home. Plus some do not want to have the thought of Gramps dying on the living room floor. Remember you have more than one patient(per se) when you work a code. You have the soon to be dead guy and those left over to go on. Some people are good at breaking the news that their loved one has died but others are not.
  15. we were taught about it and it was drilled into our heads DO NOT ACCESS THAT SHUNT DO NOT ACCESS THAT SHUNT DO NOT ACCESS THAT SHUNT But in the end and the patient is coding and the only access you have is the shunt and you are 30 minutes out from the nearest hospital - you can bet your rear end that I'm gonna get orders and instructions to access that shunt. Let the patient crash and code while they had a good shunt - not on my watch.
  16. Let me clarify Dust Devil man, All those criteria are relative and every code is different. Those rules were never set in stone they were guidelines in my "head" only. Every code is different and I can't remember the last time that those were followed to the letter for me. It is situational. The only time I didn't work the patient were the times that they qualified as dead, rigor, decomp Every code got at least some measure of ACLS but you can bet your butt I was on the phone with med direction on the ones I wanted and thought we could end early. "just remember you can't kill em any deader"

Sarcasm - Now Dust you must not know our legal system very well - don't you know if you find the right Schyster (sic) lawyer that they will sue anyone - Sarcasm If they sue it will probably get to court but the homeowner will lose. I just hope that the fire department brought plenty of hot dogs and marshmallows.

  19. great advice Jake Just make sure you learn the basics in EMT class. It's one thing to get it all done in one foul swoop and in one long continuous process but nothing takes the place of experience. that is why I say to have a delay between emt to emt-p class as you get that field experience. You will be a better provider for it.
  20. Get your EMT - if you are younger than 25 then you might find it hard to be covered under the services insurance but then again it might not be a problem at all. Do not go directly into paramedic school after you get your emt. you need field experience to supplement your paramedic. But if you do decide to to into paramedic directly after emt then by all means get a lot of ride time if you can.
  21. UHHH I think your protocols would say YES!!!! but I don't see them so I'd say yes Scapula pain - I would collar her and lsb him. ETOH yep she'd get a c-collar Moderate damage on the car - YEP Airbag deployment - you didn't say anything about seat belts. If airbag deployed and she wasn't wearing a seatbelt which I cannot believe that someone with ETOH wouldn't use a seatbelt, then heck yes I'd collar them. We all know that Drunks are too STUPID on scene to know what's best for them. What did she hit?
  22. I had criteria that I put into play a long time ago while working in the field regarding cardiac arrests 1. Peds codes always went to the hospital if we started working on them. 2. Witnessed arrest and cpr in progress with less than a 5 minute drive to the scene - work and transport 3. Any arrest without cpr and greater than 10 minute drive to the scene - work on scene and call if appropriate 4. Trauma codes - work to the hospital 5. Any time it took me 20 minutes or more to get to the scene for any type of arrest - work on scene and call if appropriate 6. Any time it was over 20 minutes to get to the scene, no cpr or interventions done by first responders and the patient was asystole - didn't even work them other than quick look. 7. Cold water drowning - above criteria go out the window Come on, if it takes 20 minutes to get to someone in asystole with no cpr or interventions done then they get a ride to the funeral home unless it's cold water drowning. If the drive time back to the hospital was less than 5 minutes - no lights and sirens Drive time greater than 5 minutes - l&s as appropriate Of course all my criteria would be backed up by a call to the ER Doc and his advice requested. Most time's he/she agreed with me. There are exceptions to every rule but those criteria
  23. CSR I think you are wrong but your opinion is yours. The homeowner said that he did not know that he needed to pay dues. He says he never was told. That's not neglecting to join, it's information that he did not get.
