  1. ...hmmm...I dont know that there is much you can do. Call dispatch update about your partner ,(PRAY), triage, and reassess reassess reassess until back up units and choppers arrive (that is if choppers can get to your location). :?

Out here for training we have an in-service where w spent 2-3 hours covering various different topics ECG's, meds etc...I am also lucky enough to be able to spent some of my hours on call at our local ER doing various work around the ER, X-rays lab (etc..) exposing me to specialties I would not normally be around or even familiar with. Local docs also allow us to assist with most of the procedures done. Because we are so far out we are able to sit in once a week on the Telemedicine link and learn about the newest treatments or new research found by docs in the city. It's a live 2-3 hour "in class" type approach. It works out great. Learning lots and being exposed to quite a bit. Being so far out tho I wouldnt mind spending more time on EMS topics but thats what we get for being where we are.
  6. oops....so I was I was saying..Not only do we have long transport time to our local hospital but the hospital that we do have is very basic, so all our more serious patients have to be take to a trauma centre thats just under 2 hours away. If its clear day then we can air med evac which works out great if they can fly out.

  10. Do I use automatic BP cuffs?.......well yes and no. We do carry the Zoll cardiac monitors who give the option of using automatic BP cuffs but I dont normally use it. The first base blood pressure that I take I always take manually that way I know its right. For long transfers I will sometimes use the automatic BP cuff because its easier but will not rely on it. If the numbers look like they are right and my patient is happily sleeping and the rest of my vitals look great then I will use those numbers otherwise no. I have used those monitors hundreds of times and about a quarter of the BP's I received from my monitor were completely off. You take your initial BP which looks great then 5 mins later you take another one and still looks great yet your pt looks like crap and is obviously not doing so good. I would have double check my BP manually and BINGO my BP is going down the drain. The monitor was wrong..yet again. I rather rely on my numbers than those of a machine if I can help it. Even I have it available in the trucks I dont usually use it.

  12. I'm not sure I have actually yet dealt with my first loss yet. It was a code for which I have actually posted a scenario online. It was a good call at the time but have recently been really second guessing my treatment for my patient. I have talked to our medical director and was already told that with the amount of knowledge I have, at my level of training there is nothing that I could have done BUT now I've realised the true lack of control that we really have. You can't save them all, and even the ones that you sometimes can save still die because it was their time to go. It's tough Healing is a slow process.
