Summit, Comments: IM - should be added IV - I assume you allow bolus in that, with respective chest assessment q volume CPAP - do alot of services really carry CPAP? 3-lead - meh, with that drug list you should really have the education of 3-lead interpretation. 3-lead means very little anyway. 12-lead - you can do them, just not interpret Atropine - is that for symptomatic bradycardia? or just cholinergic toxidrome? I would be open to treat for symptomatic brady Epi - we give it for croup here as well. If you like that as an option Dimenhydranate - I be open to this NTG - I assume first time ntg is ok with the line? for chf/pe as well? Narcan - I still don't understand why narcan is such a big drug elsewhere. We rarely use it, and those times we do it is for known/suspected acute opioid use (i.e. peds) or as a reversal for failed facilitated intubation. But anyways.... Looks good.
  4. So sorry, you are an EMT-B who is going for your EMT-P? But you have various other certs. and don't think you should have to do a 2 year program because of it? Doesn't matter, different fields regardless of similar skill sets. In Ontario, there are no bridging programs for RT/RN or even MD's I believe. They all would have to start from scratch. 2 year PCP + 1 year ACP + 1 year CCP, with various road time in between. Will it be "easier" for them? Yes, very likely, but that doesn't exempt them. I personally know of several example's of RTs or RNs who wanted to enter prehospital medicine. They all had to start from scratch. But of course there will always be grandfather clauses. I also don't think that a EMT-P practicing for X years, should have to go back to school for another 2 years to "re-learn" things. Perhaps self-study modules or CME's for areas that might be expanded since they were in school. This happened recently in Ontario. They introduced a provincial ACP exam in 2003 I believe. All ACP's that had done there training before X date did not have to write the exam. All after X date and all future ACP's had to write. Grandfather clause, happens all the time.

A poor analogy to say the least....So you are comparing global evolution over billions of years to computer "evolution" which is a product of man and has only been around for what? 40 years? The point is it can't be proven. Could "God" have "directed" (which implies that evolution is leading to man, which it didn't) man's evolution? Sure, but that relies on your respected faith and belief in a system that cannot ever be tested or proven. Natural selection at least proposes a mechanism that can be tested, found fault within, etc... Exactly, but it is written by a man. A story.

  7. Sorry, just a couple of things... - Afib wasn't new right? SOB from hospital wasn't new onset a-fib was it? - Albuterol is that new? If not COPD'er probably, could precipitate a new onset afib? That plus any previous LV dysfunction/current AMI = CHF? - Why and for how long was he on the lasix? What is his dose? Just for ascities/peripheral edema ala liver hx or did he get "water on the lungs" - HR and BP look good, I would tend to think if this was AMI that it likely isn't a Inferior MI with RV involvement. Low and slow vs. high and fast for lack of a better analogy. Especially after the NTG. But it is very likely LV ----> RV failure. As ERDoc stated. If it was purely a RV prob chest would likely be clear but with the JVD/ascities/edema.

Nobody has really commented or challenged some of the comments I or others have made of contention to the whole creationism theory. All I ask is for an attempt at proof. But before the creationists and theists wax prophetically....I ask you this... She can do whatever she wants, but she has chosen to tell me. Evolutionary theory? There are other planets, galaxies that "think" different, well you guys wouldn't be able to contemplate but... "in reality" she knows. Evolutionary theory seems to be the most plausible now, just as religious theory was "fact" not to long ago. Of course I wasn't there, I cannot comment. Look at the last 100 years peeps, seriously, phenomenal what we have accomplished, I'm boggled. But don't let that jade you...
  10. Nobody is saying that. "Better" medic is relative, I am sure that what I regard is a "better" medic is different than you, and is different from everyone. Education and graduation at least lets me know that you met some sort of "higher" requirement......or minimal. That says little. Stop trying to save a life and get educated. How could you possibly think that in a field as "important" as your's that not having a greater mandatory education than what currently exists is cool? 100+ hours? SOLID EDUCATION. Honestly.....Doctors need what 7+ years? Why shouldn't we need 2....AT A VERY VERY MINIMUM.......
  11. For lack of a better universal cultural example... We live in THE MATRIX. I agree. An omnipotent, omnipresent, omniscience being overlooks the universe, and created it. I agree. I would hope that EVERYONE agree's to the above. If anything, due to the fact that we cannot currently postulate, fathom, let alone prove otherwise. And trust me I hope, but don't count on, it will be shown in my life time "where we came from". These are ages old arguments. It would be ignorant not to think so, these are questions that have endured since our species existed. I would never ever doubt the impact that religion has on our lives. To some extent, no matter how loosely interpreted we are all religious. But that is just the human in me, I know no different. Don't believe the word of man, for he was, as you are, a thinking/expressive/rational person. A persuasive species we are, likely one of infinite. We are likely "descended" from common ancestors.From apes. From fish. From slugs. We share DNA. That is the most probable theory right now. But as I have said, paradigms shift. I dunno I have only been around for 20+ years. I only know of television, the Internet, DVD's....I cannot even think of what people 90+ years old think today. Oh what a wondrous world we live in? Or do they? I wish I knew.

  14. When I first read this, I thought you said that a "Jesus" is referenced in all respective documents that the majority of each individual religion is based on. I was like "whoa" perhaps a unifying principal in religion, not that it proves the existance in a higher being, but still. I admit I am not even rudimentarily schooled in world religion. I now know what you mean... Did Jesus the man exist? Probably. Did L. Ron Hubbard? Yup. Did Marshall Applewhite exist? Yup, but not anymore... You are missing many, many points here Angelkiss. I will tell you this.... I am born from the great creator. I was born just now, in your form to tell you the things that are true. All those things that you saw vs-eh? writing before, that was the great creator's plan, as it is my plan speaking to you now. But know this, I have just been born to this earth now. The creator of the universe sure set me up in a shitey pad though.... Do you believe that? Why not? My intention isn't to degrade this post. It is remaining remarkably civil. After I read UMassEMS18's post today I thought of a classic argument that I remembered from a first year philosophy class in university. I couldn't remember it, but the magic of google put me in the right direction... Pascal's Wager. 1. One does not know whether God exists. 2. Not believing in God is bad for one's eternal soul if God does exist. 3. Believing in God is of no consequence if God does not exist. 4. I think even the most staunch atheist, when it comes right, right down to it have to concede that there very well could be a higher power who created the universe. You can't deny the existence of something that cannot be shown to exist.

  19. For those who choose to not accept evolutionary theory, and it apears we are concentrating on the Christian faith, what is your take on Hinduism? Budism? Islamic religions? Scientology? Tom? Talk to me man... :wink: Today I could write a document (well I couldn't), commenting on tsunami's, shark attacks, and space shuttle disaster's and extrapolate them into a religious context. I have ZERO doubt that it would be accepted by more people than I would think. Follow me my children... I don't deny that the bible tells of certain events that happened (I have not read the bible). Floods, whatever, happened. They happen now as then. However the bible, as my hypothetical book, has made these events to serve a "higher" purpose. It caught on, was accepted by society at that time and still exists now. Initially I thought Scientology was a religion based on science, alot of people believe it, but it is pretty wacky, but no less valid than say Christianity. It just hasn't been around as long. I most definitely agree. UMassEMS18 - The text you reference appears to be in favour of the gradualism theory within evolution. Gradualism vs. punctuated equilibrium, that is another debate, and one that I don't remember all the key points on either side. An incomplete fossil record, to say the least, limits us wholly. How it presents though is a gradual march to Homo sapiens and to me (correct me if I'm wrong) implies a purpose and direction. A means to an end. That I don't see that... time, space, and chance prevent me. So who wrote the bible? It is 4:30am, July 31, 2005. It is 2005 A.D.....C.E.......whatever. Religion and faith play a role with every second that passes. Everyone feels it. I don't deny that. But putting many of your eggs into a basket that cannot be challenged, cannot be falsified, cannot be proven nor denied, seems a little fruitless. Turn around, close your eyes, lean back and fall, I'll catch you...Right?

  21. Good rebuttal. EDIT - I refuse to let this thread die without at least some pseudo-coherent arguments for the creationism/against evolution side. This isn't being arrogant or insulting or anything. Don't take it as such. Just asking for your opinion regarding why you believe what you believe. "Believe" is the key word here. Many others, including myself have given reasons for our argument, state yours.

  24. Exactly. Well said. Ummmmm re-READ some of my posts. Look at some Hominidae phylogenys... I don't even know what to say... I think that this debate is great. Medicine is an academic field. Disagreement is at the core of academia. Having faith/religion is great, a fundamental characteristic of "being human". Personally I am not a very religious person, I read books on religion though and appriciate what we as a species (and perhaps other species) have created. Like I have said, I do hope that that higher being rolls up in a 5.0 on my death and shows me the error in my ways...That would be awesome...But I doubt it is happening. The evidence just isn't there. Higher beings are a creation of the human mind, maybe more species too, who know's. I ask the ever increasing number of people who are submitting to not accepting the theory of evolution poll to at least give evidence for your stance. Beyond the "I believe" argument.... Again it is based on faith, cannot be tested. Again, the universe is created and destroyed every nanosecond....We live in the Matrix....Sure, but we have no way to test it. The world we live in? Or all worlds, every planet in the universe, likely billions and billions...Why would an omnipotent, omniscience, omnipresent higher being care about us? Again we are imposing human emotion, human thought processes on this higher being. "God" or the higher being if it does indeed exist, at least give me that it isn't a human. My $17.50
