  1. Sorry man, im not sure what your asking. If its about GTN my answer is no. The pt is denying "chest pain" pr se (even with the shoulder tingling) but the inferior on an already unstable patient even with a B/P of 160 would be a no go zone to me... and thats a pretty big statesment as going by the book (our book) he is indicated for it, so i'd withold and keep frank-starling happy. Also, at 50 y/o i dont think he is likely to have well devloped significant collateral circulation, so there is a chance the GTN will have minimal effect with a whole lot of risk attached.

    Treating with aspirin and diesel is more than appropriate, he either gets drano from the local hospital or a chopper ride to a PCI lab.

    what about us giving the draino bushy......some of us can do that now....

    I agree with your statement Bushy, hence why I did not administer any NTG on this call. Just curious as our protocol as would dictate to give it, and it's our discretion to not administer as appropriate. As Paramagic said as well, the risks outweighed the benefit in this case. However, I did read a while back about some testing done that came to a conclusion that administering NTG increased the v-fib threshold, which is why I have posed the question.

    I haven't put defib pads on every STEMI patient in the past and I'd say I transport around 30-40 a year. This is the first STEMI patient I've had that has gone into v-fib. I'm not sure if it will change my practice as it only takes seconds to put the defib pads on someone. It certainly wasn't a hindrance on this call.

    protocol here states that the Pt must have 'chest pain'....seeing he has no chest pain he dont get it, how ever if you feel that he needed it you could attempt to steer him to telling the truth about the chest pain....'does it hurt here" when point to a spot on his chest....voila....chest pain

    I might hold off a little on the metalayse if the Pt went in to a Vfib arrest..........gee the stuff just about kills em any way.........

    I will take it as a joke...but a very unprofessional one at that..........

    as a "manager" (what you profess, but still have yet to show) you should realise that statements like this only denegrates women in EMS.

    If you are happy to place a "boobs" out pic for your avatar and then even joke about using clevage as a way to do things just show how out of touch you are. To even joke like that on a public forum is inaapropriate and so easily offensive to others. some how i fail to see how you have any role in a mangerial capacity. and if you do then it speaks so well of the company that employs you.

    So you see that it is fime to make sexist and degrading jokes on this site but then find it inappropriate to transport a seeing eye dog, wont have a firearm in your ambulance (not a BUS, I hate that term) for ANY reason, but then feel it is ok to transport a mobidly obese patient unrestrained.

    just wondering as to where you got your management qualifications... I know if i said and did the things you state that my time as a manager would be very short.......and the 120 officers that are under my care would not have ANY confidence in me.

  4. My friends have been going every year for the past 6. They leave about mid january and return end of march. They go to Brisbane (spelling) area and they are there for the tennis opens. They rent an apartment as it the cheapest way to go. If you want any info just let me know and ill find out for sure what they do.

    gets pretty hot that time of year, especially in brisbane (and humid)...the tennis is in melbourne at that time

    best time is round march april may or september october if you want nicer weather

    if you want to ski, come in our winter

    you want sun and heat....summer

    if doing the tourist thing, early autum or mid spring would be the best for comfort, but not everything is happening at some times

    like i said depends on what you want to do/ see

  8. The problem I run into with those posts Craig is that every time there is a shitty story you say something like, "I'm would never work in the shitty U.S." You seem to like to imply that every bad thing that happens somewhere, happens everywhere, and that Australia has no stories like this.

    I've never worked in a system like this one mentioned, and I would never do so. These situations are just not that common.

    It's truly hard to believe that you really believe that out whole system works this way. And equaly hard to believe that you're going to pretend that every service and every posting in Austraila is just another perfectly professional, happy day at Wonka's chocolate factory.

    I'm not trying to be shitty, and have nothing but respect for you, but when nearly every post comes with a "I'm glad I work in Eden and not with you ignorant assholes" disclaimer, it get tiring after a bit..

    thanks dwayne....never implied that anyone from the states were ignorant asshole, you seemed to put that label there your self...ruffems placed that it would be a sick day, or what ever, IT IS draconian that the worker has to use their time to payback for time lost due to mismangagement....shit it happens here as well to some extent. I have many friends that work in the states and I even enjoyed the time i was there. but the grass is not greener there to what i have here.... Like i said I am happy working here in rural aus and not having to worry about wether my employer will 'dock' me for being to exhausted to perform after doing excessive overtime

    Ok so sick day wasn't the politically correct term but they did get the opportunity to take the day off using their PTO time. That constitutes either a sick day or personal day. It's all semantics though.

    And where did you get your knowledge that being exhausted was workmans comp instead of a sick day or a personal day?

    The short answer is that there isn't any one definitive law that says how long an employer can keep you at work, especially in medical transport. You see, since no one has been able to successfully realize the difference between a legitimate emergency and shoehorning someone into an ambulance to take the to rehab, the laxity and loopholes that were created to allow units to operate unrestricted in an emergency allow for those wonderful entrepreneurs running XYZ ambulance service to keep their employees at work as long as they can. No state agency wants to say "EMS has to have 8 hours off between shifts" because, then, what if a major MCI drops? Crews might have to work for 24 or 36 hours straight. And thus, so long as we define routine medical transport as "emergencies" "emergency" procedures will allow employers to make a buck. Ain't it fun?

    nsw state ambulance officers award has it written that all staff are required to have a minimum break of 10 hours between rostered shifts.

    so if i work 6am to 6 pm the earliest i can then start my next rostere shift is 4 am (but it would normally be 6 am)

    On a side note, as a supervisor, I would never force a medic or emt to work past the point of exhaustion. I would tell them to go to the crews quarters and take a nap. I would then hop on their ambulance and work for them for 4 hours or some length of time. If I was unable to do that due to stuff I had to do then I'd find someone to come in for them for a short period of time or give them the option of taking a sick day.

    Exhausted medics are not any different in risk I believe than drunk or drugged medics. Same risks different catalyst.

    when does exhaustion ON THE JOB lead to a sick day.......if anything it is workers comp,as it is not you fault that the amount of overtime that you have been required to work attributed to the condition that you now have....

  10. Had a similar call here, young med student blows his brains out (literally almost all brain matter splattered on ceiling and wall - GSW through mouth); EMS called the patient dead at scene, family sued because they did not try to save him.

    and that goes to show the world how stuffed up America is with the litigation and blame someone else state they have gotten themselve into...........

    what a blood stuffed up system, hose jockey tossers are just that, why on earth you would need to send them FIRST on a medical call is beyond me

  13. glad that cant happen here, i feel that seeing she had a reprimand letter first and then after speaking to the media about she got terminated, well wrongful dismissal suit is there.

    PR for the company is a nightmare as if they persue it and she appeals or sues then all the BAD styff comes out againts the company......

    too much O/T putting lives at risk

    dangerous work practices at XXXX company
