im not sure i quite understand.....

TCA OD produces a wide QRS tachy that you slow down by blockading the sodium channels further in orde to dampen the effects of the adrenergic stimulation? So you end up with a wide QRS rhythm thats not so tachy?

I thought the TCA overdose caused excessive catecholamine buildup in the synapses because of its blockade of the amine pump and the arrhythmias start from the excessive adrenergic stimulation. The lignocaine is used to slow down and reduce the number of action potentials generated by blocking the sodium channels therfore stabilising the membrane, not by acting directly on the catecholamine levels in the synapse nor the adrenergic receptors????????????

Ye Gadz! it took a while to read all this, and im sorry i didn't pay more attention, but as my young friend timmy has some people confused about EMS in Australia

000 services are covered by State government funded services. In these services and broadly speaking there are roughly 2 levels of provider. Paramedic which i guess you could say is an EMT - I or there abouts, and some form of Intensive Care Paramedic.

there are no private companies or fire departments doing 000, except some first response but that is limited to some select fire departmnents and is only for MVA where they are first on scene, and cardiac arrest due to shorter response times. In areas where fire is doing this the standrds are set by the states service or government and there are clear boundaries as to what the fire department can and cant do EMS wise

Now EMT as aqualification varies quite a bit from place to place, but in general terms it is developed for industrial organisations and is sold as a course by registered training companies and in most cases the name "EMT" is just that - a name. Organisations are not required to have this level of training in their workforce, but there are advantages in insurance etc. It far exceeds what is the minimum requirements occupational health and safety wise.

A few of the EMT type courses i know of are trained to use the drugs found in remote areas. EMS in remote areas is generally covered by Royal Flying Doctors Service (RFDS). Remote locations also have a kit that contains some of drugs and interventions that can be performed by a trained first responder of some sort, and with the permission of the Doctor who is on the plane flying to their location. For example anti venom can be administered under medical control to a patient at a remote location.

EMT type courses in aus go a little bit further into this to allow some advanced airway management procedures and fluid resus, but is usually under the control of a doctor from RFDS, or an onsite doctor employed by the organisation - its placing a higher level of first responder at locations that do not require or cannot afford a frontline ambulance paramedic

The other place you see EMT type people is at motor racing, sports etc Who again are trained to a level a bit higher than advanced first aid, but again below ambulance paramedic

So yeah some can do tubes and IV's, none do 000, and EMT is just a broadly used name.

Its a shame, i work for a company who does event support as en EMT type person, and we get chided very harshly by a lot of the state service guys from being wannabe ambo's, and trying to take their jobs - yet they will also whinge when its them at the football handing out icepacks instead of us - really bad attitudes toward anyone other than themsleves

Dusty, as to officers taking ambulances home - usually done by people who are rostered on call. In the case that the ambulances in station are all busy and a job goes down, they page the on call and 2 cars with one officer in each go directly to the job and either await a car to come and take over, or do the Tx and Rx themselves

Best evidence says that the ZAP has a lower 30 day survival rate than CPR. Literature is as slanted as any other opinion, BUT it holds far more value in levels of evidence than expert opinion

Thunderchilds decision for CPR is more than valid and we must remember that pre-hospital success is far from patient viability. If its true that the 30 day survival is lower, than it should not matter that it works well for us, it does not work well for them, and thats the bottom line

I helped a vet do CPR ona horse once, while i did compressions (literally laying the knee in) his assisstant did mouth to nose and he shaved this horse with an electric shaver, pulled out the gel and shocked the crap outta it with a lifepak 10!!!!

Well, in an effort to buy a small "jump bag", i ended up with a stat packs load'n'go for 80 bucks AU (instead of the 160US its worth - thanks EBAY!) and the thing is just massive - far bigger than i thought it would be, so i have done the only thing i can do, and that is crammed if with as much stuff as i could.

Bear in mind that this thing gets used every weekend, and i dont have to pay for its contents, and when it does gets used it quite common to have to walk 5 or so kms into the scrub and sit there all day, so you have to have a lot of basic dressing with you

ok, you young uns and you older badgers.....i am actively involved in cadet program as an adult, so believe when i say this, it has merit behind it..

Its not about protecting, you, the maturity of that young people may or may not have, the type of stuff that cadets do, the service they are with or whatever....

Its about what is legally, ethically and morally right. Arguments for that include are some kids are mature and enthusiastic and may be better than a lot of adults, and i can dig that, but it is still a null and void argument because as a minor you are not legally responsible, accountable nor professionally liable for your actions, and the dropkick adult is....

You cant plan ahead, you cant decide "hey son you better stay here, this sounds blike a bad one", you cant say that we sit around doing transfers all day. Emergency services is built on analysis of risk - the risk that something will go wrong, and while it might not go wrong very often when it does, its a shit storm, and i gurantee you that there will be a great ruckus afterwards when people start screaming "why was a minor there?"

EMS juniors is a great thing, but it does not matter how mature we think they are - god knows i know some awesome 16 year olds, but in the end its what is legally, morally and ethically acceptable, and services who allow minors to be front line personnel, even as ride alongs, are not thinking about their level of service

I think ems cadets or whatever is a fantastic thing, and there are many cadets i have met that are really good at the first aid they provide. Out of all the people here, Timmy is the man to talk to. I would guess you are a cadet leader mate???

Show the kid some help and you never know, he might turn into a good EMT down the track, the fact hes asking about it now is a good sign, give the kid a hand an he might be able to learn a thing or two BEFORE he starts his classess
