  2. Will be thinking of them and their families. I was always argued about medics in the back wearing seat belts and how they limited movement around the patient. I myself had been tossed around a bit, like probably everybody else here. But coming back from a call they should be used. IMHO.

  4. I guess I'll go with the crowd that says to not lift the weight off. You lose any tamponade and he'll bleed out quick. The cold weather might be an asset. Keep him warm but the cold may help if he is somewhat hypothermic. High flow O2 is a must. Since you can't assess the extent of lower extremities you assume the worse. Can the feet be exposed to check for circulation? Can someone dig out enough to check? That's about all you can do until ALS arrives. It goes without saying, two large bore IV's (some will argue LR with both, or one LR and one NS). Use MAST if available. I was raised in the oil field. My father was an oil producer for many years. And I have seen those kind of injuries and almost any kind of injury you can think of associated to the oil field. I was going to get a bug shield on my truck that said "Fourth Generation Oilfield Trash" but my wife (my first one) wouldn't let me do it.
  5. I don't think we ever had a problem with being turned around and not being needed. Most officers we worked with would not turn us around if there were even the slightest hint of an injury, no matter how minor it appeared to be. One halloween night we had a MVA where when we got there was up walking around and refusing treatment of any kind. And the gal with him kept trying to convince him of at least being looked at. He kept rubbing his neck and said he only had a little headache that he had before the accident. But when the officer started talking to him about DUI he decided he needed to go to the hospital. Sure enough, 3 c-spine fractures. Guess he thought if he went to the hospital he'd get out of the DUI. Wrong...Actually went on another call that concerned him. He was playing cards and one of the kids threw something and hit his halo. Guess when that happened it actually knocked loose one of the screws going into his head.
  6. I've also noticed that "ice cold" applicants do cause more pain. That's why I've always suggested cool compresses. One note I'll make from when I was doing my sport's medicine, It's cold for swelling, heat for pain. Apply what needs to be addressed.

  10. There is one precaution that I was told by an Ortho. is that with the ice pack/ cold compression be alert for muscle spasms. If too cold or left on for too long it could cause contractions followed by spasms. Just a little FYI.

  15. Some districts may have to depend on younger personnel to be adequately staffed, especially volunteer services. There's just not enough more mature individuals available to volunteer or staff a service. I think the youngest I've heard of was 14. But for the most part it's been at least 16. That's been a while back. Now I don't think there are any near me that are under 18.
  16. I have a 12yr. old niece with cystic fibrosis. She has had Methicillin-Resistant Staphylococcus Aureus (MRSA). Luckily in the last year she has tested negative for it. But any time she had a cut, abrasion, moderate to severe bruising, etc., she had to use a special sanitizing gel to keep it from getting a severe infection. One of her older sisters passed away from CF when she was 9. But I don't remember her ever having MRSA.

  20. I have. We had 3-5 local patients that were positive for TB, and at least two of them were active cases. So we had a few extra in-services and were vaccinated because of them. There had always been a history of TB patients around there. In matter of fact there is an old TB sanitarium there left over from the '60's. A hospital there converted it into class rooms and offices.
