The doctor seems to be saying that though the face is hollow, your brain refuses to see it as such based on your previous knowledge that faces are never hollow.

How many the's?

This may help explain why motorists say they "never saw" the motorcycle or bicycle or pedestrian or animal they hit - if such objects do not fit into the frame of reference of what they expected to see on the road.

This also might be why we're so reluctant to believe that someone doesn't know what they're talking about if they seem normally equipped with a solid rather than hollow head.

Or heart, as when a beloved starts to behave abusively and the lover remains "in denial."
