  1. Welcome Sharon. You will come to enjoy this site as you progress through your class work. But yes watch out for "the kiwi". Hes a very unique specimen that is easily startled by loud noises. But in the event he should retreat to his yellow ambulance. You can easily coax him out with a good basket of fish and chips. The kiwi loves fish and chips.

    I cant speak for everyone but I believe most will agree. Theirs no better group of people. These medics and EMT's are some of the best in the business. And if they dont have an answer to your question they know where to find it.

    I always called it a medium duty. Whenever I thought of type I I always thought of a box mounted on a f350, chevy 3500, or dodge 3500. Its been so long since I have reviewed EMS ops can you enlighten me as how they classify each ambulance.

  5. My supervisor would die to get it back in service. But the nearest service center is 6 hours away and they dont want it gone for a month or shell out the hundreds of dollars it would cost to fix the short the kills the battery after turning it off. Its our most fuel effcient , smoothest riding, and fastest off the block. But its also the ugliest excuse of a vanbulance I have ever seen.

    Comes with the territory. Welcome to the city and congrats on your written.

    Its the differences in pt care and debate between different views that both enlighten and improve us as health care providers. And as a person as well.

  8. MFI (Medicine Facilitated Intubation)

    Its same principles as RSI however it does not use a paralytic. It just uses versed and a long lasting sedative (cant remember the one our protocols use but Ill be glad to check once I'm done with clinicals)

  9. Saving someone from a cardiac arrest is just doing our job. Outcomes post arrest are grim. I've worked 2 arrests in the last week, got a pulse and a pressure back on both of them, only to find out they passed a couple of days later. I sometimes wonder to myself...Have I done them any good at all. I dont think I have. I prolonged thier lives long enough for family to say goodbye. I guess that should be enough but it isnt.

    Even though you might not have changed the patients outcome. You did indirectly change the outcome of how the family says goodbye to their loved one. And if a family member has time to say goodbye to them I think that might make it worth it.

  10. Open up the lines and titrate to keep bp @ 90 systolic.

    Initiate MFI to secure airway

    Needle decompression on the affected side (3rd intercostal space if memory should serve me right)

    Place the combo pads on in route and be ready for cardiac arrest

    Call into the level 1 trauma center and alert them to have a surgical team ready

    Be ready for vomiting of blood

    put on safety googles/ glasses

    do I have an engine onscene where I can take rescue crew with me to handle ventilations?

    if not ill steal a cop and have him do it

    also I didnt see if anyone else asked this but....Has this guy been searched for weapons himself.

  11. Perhaps I need to better calrify. Under texas penal law section 9.41 states a land owner can use reasonable force if summoning law enforcement is not a viable option or when the actor believes it is ultimately necessary to prevent or terminate trespass. Necessary force can go as far as firing upon trespasser or can be a simple as physically removing said trespasser.

    However texas penal law section 9.42 states

    A person is justified in using deadly force against another to protect land or tangible, movable property: (1) if he would be justified in using force against the other under Section 9.41; and

    (2) when and to the degree he reasonably believes the deadly force is immediately necessary: (A) to prevent the other's imminent commission of arson, burglary, robbery, aggravated robbery, theft during the nighttime, or criminal mischief during the nighttime; or to prevent the other who is fleeing immediately after committing burglary, robbery, aggravated robbery, or theft during the nighttime from escaping with the property; and

    (3) he reasonably believes that: (A) the land or property cannot be protected or recovered by any other means; or the use of force other than deadly force to protect or recover the land or property would expose the actor or another to a substantial risk of death or serious bodily injury.

  12. The above post makes me think two different people wrote it. You respect a people but if you see them on the farm you will shoot them. Do you even realize how messed up that is?

    I feel for your rancher buddy and every other land owner that suffers property damage.Violence is not the answer.

    To be honest. I didnt know what to do when she told me she was illegal. For a moment I was very tempted to turn her in. But I couldnt because was my friend. Some run of the mill man that just crossed the rio grande. Different story

    According to Texas law if you are trespassing on another mans property. You can be shot at if the land owner gives warning to the trespasser and he does not acknowledge the warning. (However the owner must cease fire should the said trespasser runs away as if he continues to shoot and succeeds in killing him its murder)

    If you saw someone on your land and didnt know their intentions what would you do. Waltz right up to them offer you a cup of tea. Hell no.

    For the record people I will do what ever is deemed necessary to protect myself, my family, and my friends. If that only means firing off a warning shot stating I mean business. Smart on him and less headaches for me. But if he pulls out a gun or continues to advance toward me I will act.

    have 2 IVS ready to go once he begins entering decompensated shock

    manage any bleeding and cover any sucking chest wounds with a three sided occlusive dressing

    Prepare for a L sided needle decompression if indicated

    *EDIT:TKO....Until signs of decompensated shock are present and titrate to maintain a BP of 90 mmHg systolic

    Go ahead and place on cardiac monitor as well. Also clear airway with suction
