  1. Here is my two cents worth on this one,,,, I have been in EMS for 16 years and all these years I have done nothing but volunteer, That is my personal choice. I love EMS cant imagine my life without it . But I do understand the points of view of those that this is their carreer. But I dont think that is fair for the carreer people to not want volunteers around....well I say in this world we all need to pull together...everyone has a place in the system either way paid or volly ... We as a system in whole are our own worst enemies We bicker back and forth between each other. Just because you are paid dont make you any smarter or better than me because I volunteer and because I volunteer that dont make me any better than you because you get paid. I had to go to the same training you did, and last time i looked the states or the National Registry just dont hand out cards to people just because they want them. And to say that because I am a volunteer that makes me unprofessional is just wrong. Professionalism is not based on my payday or volunteer status it based on my attitude.

  3. I agree canuck, our protocols use Glucagon as a last ditch effort to oral glucose or D50. Takes too long for it to work IM. That's the only thing I don't understand. I think that it is great that someone in a long term care facility would take the initiative to go and agressively treat a patient under their care. Up in boondocksville NH it's hard to even get these places to recognize cardiac arrest!

    Cos ......To answer the Glucagon question our standing order at the NH for low BS is give it first if unresponsive ....I aggressive treat my residents because unlike some of you guys that think that Nursing home nurses are stupid and lazy I am not...I was not trained as nurse or a EMT I to not do what is needed to save a life ...I do what needs to be done when it needs to be done. I work at a boondocksville NH and honey you can bet your sweet ass that i know what cardiac arrest looks like and how to initate treatment on it.

    here is the full story

    I work in a long term care facality as a nursing supervisor ........i was on the phone with the md getting the orders for all the interventions when the first amp of d50 was given followed by the 500cc bolus and i was getting ready to push the second when the squad arrived to transport the patient to the ER...when they arrived on scene 20 mins later they were questioning why i gave the medications and didnt wait on them to arrive. I was wondering if anyone else would question that or not ......hope thait makes it clearer

  6. Here is the scenario:

    This takes place in long term care facility, You have a hypoglycemic patient...

    vital signs: b/p 50/pal.... resp 10 and shallow. pulse 60 and thready.....BBS clear pulse ox 95% on RA..BS 47.. the nurse supervisor has given 1ml glycogen Im per protocol and BS is now 44 after 10 minutes... the supervisor now has an order from the patients own MD to start a line and give 1 amp of D50 and a 500 cc bolus of of D5 ...if in 10 minutes the BS is below 60 push another amp of D50...you arrive on scene and have all your als procedures done for you accept placed on a monitor and NRB ..........(you arrive on scene 20 minutes after being dispatched) and the BS on your arrival is only 54 after all the interventions above have been done.....here is the question ....do you question the 500 cc bolus of D5 being given and the second amp of D50 (which was not given).......

  8. Every patient should be allowed to go for ANY reason they want, but when they ask "What do you think?" We should be able to say, "Well man, I think you need to go sit on the toilet and make a bowel movement."

    Our job is to take anyone anytime to the hospital when 911 is called...if you dont want to do your job...then GET THE HELL OUT......the remark stated above is not acceptable....shows 2 things 1. no compassion. and last but not least POOR PROFRESSIONALISM..... even the bs calls the patient needs to be treated with respect and dignity...to you it maybe bs to them it may be serious.....it is like always in the preception.......

  9. Does spelling and grammatical errors in patient run reports indicate shoddy or poor patient care? Please discuss!

    My feelings are no it does not indicate poor patient care (personally)....but it does indicate shoddy and poor doucementation on the medics part which in turn makes them look bad and unintelligent for a lack of a better word. If you dont take pride in your doucementation then yes it can make you look like you didnt take care of the patient....It is much easier to explain a clear and accurately done call sheet than it is to try to figure out what the hell you were saying when you turn in a grammarly incorrect or one that has misspelled words.....a bad lawayer can have a field day with that one....and a good lawyer will shake your hand for it...It is all in the preception....

  10. When i got home today after the call i was just so exhausted and really stressed out. I know thats all probably normal for rookies but im really freaked out about it and worried that im not going to be able to do this.

    A lesson learned is a lesson kept. You can handle anything you set your head to, you are only limited by what you dont allow yourself to do..expirence is the best teacher .....if you need to review what you did or didnt do just ask any of us here we are for the most part nonjudgmental and we will do our best to help you....i am just a pm away if you need me..... so here is my advice to you...

  11. Nope never puked yet 16 years in ems.....The thing that comes close everytime to making me puke is "snot" can handle blood, guts, brain matter, vomit and other body fluids.....but "snot" does it every time....... makes me want to puke that is

    Well up until a few nights ago I hardley ever posted in the forums becasue of various reasons but I have to once again give my thoughts on this...cause If it was not for God I would probally be dead somewhere....

    Like I said in a post the other night I was raised in a Christain home and I still carry those values and morals with me. I do not go to church on a regular basis because of an incident that happened a few years back....This incident just changed my views on the church and people in it but did not shake my faith in God. I have a very deep rooted faith in God and I know I would not be where I am now if he had not brougt me through all the things in my life that i have faced.

    Rid I do agree with on two points we are responsible for out own destiny we choose to believe in God or not...it is our choice. He gave us the free will to choose to serve him or not.

    The second point i agree with is the fact that We were never promised an easy life....when we have trails and tribulations it is God's way of letting us know he is in control and has the wheel..

    The trails we go through and how we deal with them are what makes us who we are, again another choice we can choose to see the good in everything or we can choose to see the bad and play the poor me why does things always happen to me.....Me i choose to look at each of these trails as a growing expirence. We are never too old to learn from something. God will never put more on us than we can bear....

    Well these are just my thoughts on this subject....

  14. Stay on track people and read from the beginning.

    It is about laws that are trying to be passed that enable us to withhold ANY treatment if it conflicts with our own personal religous or moralistic views..

    Once again I say.....Treatment should not be withheld due to the personal belief of the healthcare provider .....The only way health care measures should be withheld are if it is the wish of the patient....This is about the patient here not us and our beliefs....and AK you are right leaves the door open for alot of scapegoats for the world of health care....
