If it's trust, why not just use MS word or equivalent with the date along the top? If it's a password issue, it shouldn't be too hard to write up something. IT will let you export to a database as well the $20 doesn't print cards that is the training center version. The CTC version is much more expensive, but does a lot more stuff.
