ABN Forms

Does your service require ABN forms to be filled out on all non-critical/lifethreatening patients?

We've never used that form, per say, but dispatch will advise customers/patients if their non-emergent scheduled transport might not be covered and advise them of the cost.

edit: Besides, just because a patient isn't critical doesn't necessarily mean that medicare won't reimburse it.

The ABN is a form for Medicare that looks like this:

This form is supposed to be used when the medic feels like Medicare will not pay for the transport and the pt still wants to be transported by EMS. You have to write the services that MC prob will not pay for, get an estimated cost for service, then have the pt check opt 1 or 2. Its a pain in the ass but its something we have to do religiously now.

There is also a push for this form to become a standard for medicaid and private ins as well...

We only use those forms when someone who wants a pre-scheduled ambulance transport for whatever reason even though it can't be justified by the insurance. Dispatch will inform them of the cost. I've never had that situation come up...
