V-Fib Final answer

I will have to let someone else tell me what drugs to push (I know epi to start) Maybe there is a Paramedic in the crowd!
Ok I kind of pride myself on Rhythm identification... When I opened this post I said V-Fib...Let's shock. But why do I see a pattern. Great now I feel like an idiot, should not have posted. Oh well I gotta learn, I still say V-Fib, but I will still hit analyze on the Lifepak 12 like protocol says. (cheater)

So, my questions to the audience here:
How often do you wash your hands? After each call
How often do you change gloves? Every call, at traumas I layer and change before I touch the stretcher, doors to the rig etc.
Do you walk outside the patient room still wearing gloves? Yes
Do you go to the nurses' station still wearing the gloves you touched the patient with? No
Do you make notes with a pen on your gloves? No
Do you wipe your pen and stethoscope with a disinfectant before putting it back in your pocket or around your neck? No, after each shift
Do you reach in your jacket or pants pocket with gloved hands during patient care? Never
Do you consider your gloves dirty only if visibly soiled? Nope I consider them dirty as soon as I put them on
Do you clean the steering wheel frequently? Daily
Do you wear long sleeved shirts? In winter
Do you wear a jacket? In winter
How often do you launder your jacket? After each 4 day rotation
If you have long hair, do you keep it pulled back? Or, do you have to push it back while working on a patient with your gloved hand? I have short hair...Which reminds me I should be washing my touque more often.
Do you treat all patients with the same infection potential either for their safety or others? I have been working on this recently but as of now NO.
Do you wear a mask if you have a cold around an immunosuppressed or elderly patient? No
These questions are asked of all heathcare professionals inside the hospital on a regular basis.
I realize this was geared at US but I answered anyway.
I recently did a fall with a closed hip fracture, when I got to the Pt. I realized I Was not wearing gloves. I didn't stop anyway (such a sweet old lady).
Long story short, Pt. was MRSA positive and I had a cut between my fingers. Stupid Stupid Stupid.
I am now very aware of infection control and greatful I didn't have to contract Hepatitis to open my eyes up.
Great post.

Considering this will be bi-weekly and only 300 words I don't think it will be that hard. I don't expect this to last forever, I should have mentioned that I guess. If we get a year out of it that is fine by me.
-
Ya I will definatly have to keep on top of that firedoc.
I am going to be very general. The first one I am writing is on EMS in the past and EMS now.. I want people to know when they call an ambulance they are getting educated health care professionals, not a couple of taxi drivers with first aid. The articles are only around 300 words so I should be able to find enough topics for a while.

Sometimes the book doesn't cover everything.
I have never understood that comment ?
Damn thing won't play for me...GRRR

Not to be an a$$, but make sure you have someone proofread and edit for you.
Should have said, "Have any of you written one before?"
Lets make sure if we are going to put ourselves in the public eye like that, we do it correctly.
Ummm.....Ya...... Kinda figured that
I am kinda surprised I didn't get more positive support on this. I mean heck we whine and bitch constantly on this forum (me included), so I thought when a member was attempting to educate the public and try to advance the profession through a more active roll such as this, there would be huge support.
-
If anyone is interested I have taken this one step further. I will now be writing a bi-weekly article for our local newspaper on the subject of EMS. The editor said if it is exceptionally interesting he will be forwarding it to our closesed major newspaper editor in the neighboring city.
It will be nice to be able to communicate some of the achievements and struggles we face in EMS with the public.

A study was performed on 166 pediatric patients mixed post-arrest and closed head trauma. All 166 had fixed and dilated pupils.
Of the 166, only 13 were discharged from the hospital and a mere 8 of them had "productive" neurological function.(Sorry I cannot post my source I pulled those facts from a presentation I have).
Anyway the point is, no one here can tell you if you did any further damage than the mother did, but I can tell you this patient had a VERY slim, almost nill chance.
What you need to decide is if you did what is RIGHT. If you walked into a house with an unCx patient that is not breathing, and no obvious trauma you did what anyone else would do...Opened the patients airway.
I just finished a code where the wife stated "he just collapsed", we found him lying supine on the carpeted floor, first thing i did was a head tilt chin lift.
So if the story I am given afterwards by police is that the wife hit him with a frying pan and pushed him down the stairs, only to drag him back up and call us, did I do the wrong thing?
Of course not. So if I switch my adult patient for a child does that change your answer?
Perhaps you need some professional help to teach you how to deal with these feelings. We all could use some help sometimes.

OK we have all heared the stories of people walking around with broken necks
EMS/Fire/Police dispatched to single vehicle rollover. Police are first on scene, Pt. is walking around no coplaints. Police inform dispatch to cancel all other services. We return to base only to hear the next day the Pt. went to the hospital the next morning comlaining of neck pain. ER does an X-Ray....yup cervical fracture.
Can Police in your area cancel ems responce?

I have researched a bit, but have not found much info on this.
Is there any reason outside of pain control and swelling reduction that we use ice packs on fractures?
Someone mentioned that it can reduce permanent muscle damage by slowing a chemical reaction within the site. No specifics though, so I don't know whether to believe it or not.
