Your logic does not work here, homosexuality is a mental disease, I am not mentally ill, so I would not sleep with a homosexual. First of all this was the topic of the TV show the OFFICE, the night before Wendy posted this, so probably made up. But there is no greater test than who you choose to date, as to whether or not you are a racist (remember racist means that you think your race is superior, not that you burn crosses in peoples yards). You can say you are a vegetarian all day long, but when I see you eat a steak taco, i know it isnt true. And last time I checked, we do have black people up north, but I will issue a "pass" for those who live in Alaska and the Dakotas (although I have seen black people in Canada). Its a simple question, whoever you are dating now, keep it all the same except that you have not met him/her and he/she is black (if you are white, or vice versa), would you date him/her ? You havent yet, so my guess is in your heart you are saying NO, although to be politically correct, you will say YES in here.

I am not grabbing at straws. The laws vary from state to state, some have a romeo-juliet type clause where if both persons are minors, they are not prosecuted. Other states do not, if an underage girl has sex with anyone of any age it is a crime. The point is, do your report the crime, or ignore the crime. Some choose to ignore the law, some choose to report crimes. The point is that we tend to let our on values, or lack thereof get in the way.

Its simple, but you probably won't like the answer, I worked in a rural county, where the only map that had been printed was a bank map that only listed the paved roads, and was over 10 years old. Citizens were allowed to choose their house or trailor number, so it was virtually impossible to find a home, if you could find the right dirt road (most dirt roads were criss-crossed with logging roads). What did we do; we divided the existing bank map into quadrants, then the volunteer fireman and EMS folks were assigned a quadrant. It was your job to find every residence, business, church, school, fire hydrant, and potential landing zone in your quadrant. It took about a month to pull it all together to create a hand-drawn mapbook, that was then copied and distributed to everyone in public safety (PS: we found the post office to be very helpful). You can't base practice of andecotes, "miracles" or the provider's personal belief system. It has to be based of science and informed patient decision.

Transporting "to give them a chance" uselessly ties up resources, often places providers in danger (code 3 transport or HEMS), delays the grieving process and presents families who may not be able to afford it with a financial burden they may not be able to be bear.

"The best hope" in the face of damn near certain death may be saying goodbye to family and friends and passing in peace rather than being used as a high-fidelity skills lab on the side of a mountain road somewhere. The patient should certainly be informed of this and that living would be a massive improbability.

I'm not from Canada, but my understanding is you see far more fixed-wing use due to the distances involved. HEMS in the US is massively oversaturated anyway, 75% of the aircraft in our country could close up shop tomorrow and not significantly affect outcomes.

It's time for EMS to move past the "miracles" and "one-in-a-millions" and start focusing on providing good, realistic, cost-efficient care.

If we based treatment solely on scientific evidence we would never work an out of hospital cardiac arrest. I imagine most everyone in this room has worked a dead blue rigor baby, to give the parents peace of mind. We are not GOD, if this patient was in arrest and you chose not to work it, I can see that, but withholding transport because you dont think they will survive just seems wrong to me. To answer your question: Yes I know nurses, and I have worked the ER in a busy trauma center. Is Nursing work busier than EMS work, yes. Is it backbreaking labor, no.

As others have stated, you can shop online and create your wish list with prices. To answer what has not been answered: If you are actually running calls that require ALS assessment/interventions, you can bill a higher rate. But in most areas you can not bill the ALS rate just because you have a medic onboard. So you will need to research your statistics and see how often you have ALS calls and do a cost benefit analysis based on those facts.

It varies wildly from city to city and state to state. If I had to ballpark it I would say most charge around $300-500 for BLS plus mileage, and between $400-700 for ALS plus mileage, but I have heard of some that charge well over $1000. Why do you ask ? Realize that most services only collect about 50% of what they bill.

I have to disagree with not transporting. I agree that transporting him was probably not going to save his life, but it did give him the best hope. Miracles do happen. In the US, I was not allowed to NOT follow a doctor's order, so maybe I am tainted. We often choose not to "work" patients who have arrested and are not viable, but to let a patient die this way just doesnt seem right to me. Am I the only one that sees it that way ? And can I ask why no helicopters are available in Canada, is it a weather thing, or does socialized medicine not pay for that type of transport ?

If you could not talk her into going, so be it, and I am sure your documentation was very good, but you never answered my question honestly, how long were you on the scene ? What diagnostic tools did you use to ensure the patient was stable enough to be left at home. If the same patient presented to the ER, would an ER doctor release her based on those same simple diagnostic tools you used, or would the doc order some labs and xrays/scans.

Thank you for the compliment, troll in here is a compliment. I am not saying nurses do not earn their pay, but to say they work as hard as say a construction worker is laughable. I have worked ER and ICU, and yes I stayed on my feet and moving for 12 hours, but I rarely broke a sweat. I imagine these nurses are paid very well compared to the average person in any other industry. I would fire them all.

Lets all cry for the Nurses, I seriously doubt any of them are breaking a sweat at work, despite having more patients to care for, and I doubt they have substandard wages. Most nurses no longer have to draw up meds (the pharmacy does that), most charting is electronic, and they have aides to do the butt whiping. I agree that if you have a company that is abusing its employees, then unionize, but most of the bitching from people today is because they had a few perks removed, because the company is trying to stay profitable. If you have not realize how this economy has hurt collections for any healthcare provider, then I suggest you spend some time with someone in billing.

Only thing I can add, although I can not site the reference to the actual case, hospital property is either defined as within 75 or 100 yards of the physical property (the case involved someone attempting to drive to walk to hospital, made it to edge of property but not physically on property, and hospital staff refused to come assist the pt. off property). Also, a screening exam is just that (basically triage by a nurse), if no life threatening stabilization is required, there is some grey area as to how long they can "park" a patient, once that exam has occurred. One of the grayest areas is when the patient is at an outpatient facility or clinic that is distant to the main facility, and there is a closer non-related facility nearby. The patient by EMTALA should go to the base-related-hospital, but it may be in the patient's best interest to go to the closer non-related facility.

You do not know the children involved, the law is the law. Posession of marijuana is a crime in most states, you may not agree with it, but it is the law.

Correction, the Wyoming Governor does not want to force anyone out of a union, he wants to give everyone the right to choose to be in a Union or not. If you will compare the economies of "right to work states" versus those who have mandatory unions, you will see a stark contrast. If you like unions, move to detroit or pittsburg and see how unions have ruined this country.
