    Medication fridge mounted onboard Any good manufacturers.

  2. So I have been tasked with assisting a new EMS service startup company in small metro city serving roughly 95,000.

    What I am looking for is input from everyone on operations, pay, dispatch, equipment as if money is no object.

    What we have determined thus far:

    3 ambulances staffed 24/7

    2 backup ambulances

    3 stations (new build) with Plymovent exhaust.

    3 man crew (2 EMTs/ 1 paramedic)

    ABC Kelly shift

    1 shift supervisor (ALS Equipped vehicle)

    Ambulances will be a Medium Duty Freightliner chassis.(meeting the new NFPA standard)

    2 backup ambulances smaller Type I/ II

    Supervisor vehicle: F350 4x4 Crew cab

    Paramedic pay starting @ $54K

    EMT pay $40K

    Paid CDL, Critical Care Paramedic, EVOC/ CEVO, AMLS and Child Safety Seat Technician training

    Written, practical, physical agility and DOT Medical

    $8,000 relocation and hiring preference to those residing in the service area.

    The management is looking at streamlined operations without all the brass. Their thinking is they want to get rid of the revolving door and retain employees. They want each employees involved in the decision making that takes place. Their goal is to make EMS a long term career.

    Uniforms are Class A which I am not a fan of but it does look professional. I guess.

    OK folks now it is your turn. Tell me your wish list and what would be the perfect EMS model?

    Equipment, lessons learned, crew make-up, shifts, pay, etc.

  3. Thinking of moving to Pennsylvania. Can anyone give some insight as to some great agencies to work and actually pay decent? Looking at the Williamsport, PA area. How are the private agencies such as Transcare? Any information would be the greatest. Please private message if concerned about backlash. Thank you all for your time.

  4. Consider causes of such a rhythm.

    1. Electrolyte imbalance.

    2. Fever/ recent illness/ surgery.

    Verify no signs/ symptoms

    Weakness-Fatigue-dizziness-etc

    If Medical control received the 12 Lead see what the Doc thinks. After all that's why some carry that fancy ECG transmission monitor for these days. (Amongst other reasons of course)

    Not sure I would toy around with immediate cardioversion, although Amiodarone (150mg over 10 minutes) would be my choice if I had to choose an aggressive route.

    Would definitely like to see the 12 lead.

    Oxygen, rapid transport and being ready & aware of any changes in the 12 lead or patient status. I would discretely place the Multi-function pads.

    In a clockwise rotation starting from 12 o'clock position.

    1.Detection – The first rescuers on the scene, usually untrained civilians or those involved in the incident, observe the scene, understand the problem, identify the dangers to themselves and the others, and take appropriate measures to ensure their safety on the scene (environmental, electricity, chemicals, radiation, etc.).

    2.Reporting – The call for professional help is made and dispatch is connected with the victims, providing emergency medical dispatch.

    3.Response – The first rescuers provide first aid and immediate care to the extent of their capabilities.

    4.On scene care – The EMS personnel arrive and provide immediate care to the extent of their capabilities on-scene.

    5.Care in Transit – The EMS personnel proceed to transfer the patient to a hospital via an ambulance or helicopter for specialized care. They provide medical care during the transportation.

    6.Transfer to Definitive care – Appropriate specialized care is provided at the hospital.

    Sounds like a great schedule and I think you're a good one to present it, as it may confuse some.

  9. Would like to compile a list of classes available to Paramedic/ EMT's for continuing education purposes.

    So here's where I need your help. Please add any classes that you know of.

    Please post the full name of the course as well.

    I was wondering of all the classes available, which would you say are worth it.

    Here is my list to date, some I plan on taking but others are FYI.

    Some have been left out like PALS and PEPP, ACLS as most of us know them well.

    ACLS-EP Advanced Cardiac Life Support-Experienced Provider

    EP-C- Emergency Pediatric Care

    ABLS- Advanced Burn Life Support

    AHMLS- Advanced HazMat Life Support

    FRT- Farm Rescue Technician

    AMLS- Advanced Medical Life Support

    BDLS- Basic Disaster Life Support

    ATLS- Advanced Trauma Life Support (Audit)

    STABLE-Sugar, Temperature, Airway, Blood pressure, Lab work, and Emotional support. (Neonatal)

    OBSTAT- High Risk OB transport

    CCEMT-P- Critical Care Emergency Medical Transport Program-UMBC (I STAND CORRECTED)

    FP-C- Flight Paramedic Certification

    GEMS- Geriatric EMS

    ITLS- International Trauma Life Support (Formerly known as BTLS)

    NRP- Neonatal Resuscitation

    EVOC- Emergency Vehicle Operators Course

    OK so what else is available out there in the world of EMS.

  10. So back in 2004 when this was still under development, I read the proposal and from what me and others interpreted was this:

    In 2010 when this in implemented "All" paramedics national or state certified will be classified as "Advanced Emergency Medical Responder" with a few skills taken away. With more education those designated as such can obtain their Paramedic certification and get the skills "BACK" we all have been practicing all these years, and no additional "new" or more advanced skills, just the same old skills.

    Now PLEASE hear me out this was 4 years ago when this was all being drawn up and have yet to look over the NHTSA guidelines, so this may have all settled down a bit.

    I just don't see how making people whom have been Paramedics for years be reclassified and taking skills away unless they get more education to get their "Paramedic" title back.

    I can see a lot of Paramedics tapping out of the EMS business to do other things. Unless they are going to pay more for paramedics than the current pay rate.

    Excelsior RN program is looking more interesting to me now.

    Could someone who knows what is going on or someone who has the time to go read the 200 page guidelines from the NHTSA and let us all know WTF. :?:

    Oh brother.. yeah the comment wasn't supposed to be offensive. If you would like I have plenty of comments about Canadians.

    I was simply making a point there are so many other things to make ambulances safe, they must have it all. Well it's great to hear they have it making life safety top notch in which all ambulance services around the around the world should idolize.

  12. Berkeley shift schedule, which is: 24 hours on, 24 hours off, 24 hours on, 24 hours off, 24 hours on, four days off. Then the schedule repeats.

    Kelly shift schedule, which is: 24 hours on 48 off, then the cycle repeats.

    There are many variations in the Kelly schedule.

    After repeating that cycle seven times, there is six days off in row. (again many variations).

  13. Ok, so I see a Very large Southern Missouri Hospital is hiring paramedics for 10.75 an hour. Are they kidding?

    Those of you curious it is posted in the jobs section.

    Those that work there let me know how it is. Are you still carring a "thumper" and Lifepak 5?

    Just curious if the equipment is as out dated as the pay.

    I can only imagine what they pay EMT-basics.

    I am sure it's not those that work there. It's the bean counters in the 10 story building next door.

  14. If I ever get around to getting my strapping and backboard system patented it will revolutionize the spinal stabilization system.

    It takes about 30-45 seconds from start to finish.

    Not looking to make a fortune but it will make things a lot easier for everyone.

    No more velcro!!

    It's called the "The RIB" Rapid Intervention Board"

    Who knows what will come of it. There's a lot of trials coming for it.
