So I have been tasked with assisting a new EMS service startup company in small metro city serving roughly 95,000. What I am looking for is input from everyone on operations, pay, dispatch, equipment as if money is no object. What we have determined thus far: 3 ambulances staffed 24/7 2 backup ambulances 3 stations (new build) with Plymovent exhaust. 3 man crew (2 EMTs/ 1 paramedic) ABC Kelly shift 1 shift supervisor (ALS Equipped vehicle) Ambulances will be a Medium Duty Freightliner chassis.(meeting the new NFPA standard) 2 backup ambulances smaller Type I/ II Supervisor vehicle: F350 4x4 Crew cab Paramedic pay starting @ $54K EMT pay $40K Paid CDL, Critical Care Paramedic, EVOC/ CEVO, AMLS and Child Safety Seat Technician training Written, practical, physical agility and DOT Medical $8,000 relocation and hiring preference to those residing in the service area. The management is looking at streamlined operations without all the brass. Their thinking is they want to get rid of the revolving door and retain employees. They want each employees involved in the decision making that takes place. Their goal is to make EMS a long term career. Uniforms are Class A which I am not a fan of but it does look professional. I guess. OK folks now it is your turn. Tell me your wish list and what would be the perfect EMS model? Equipment, lessons learned, crew make-up, shifts, pay, etc. Appreciate all responses.

Thinking of moving to Pennsylvania. Can anyone give some insight as to some great agencies to work and actually pay decent? Looking at the Williamsport, PA area. Consider causes of such a rhythm. 1. Electrolyte imbalance. 2. Fever/ recent illness/ surgery. Verify no signs/ symptoms Weakness-Fatigue-dizziness-etc If Medical control received the 12 Lead see what the Doc thinks. After all that's why some carry that fancy ECG transmission monitor for these days. (Amongst other reasons of course) Not sure I would toy around with immediate cardioversion, although Amiodarone (150mg over 10 minutes) would be my choice if I had to choose an aggressive route. Would definitely like to see the 12 lead. Oxygen, rapid transport and being ready & aware of any changes in the 12 lead or patient status. I would discretely place the Multi-function pads. :shock: I hear there is an agency outside St Louis, Missouri in which you rotate monthly to three different jobs. Paramedic one month, Police Officer the next and then firefighter for the third and final month. Apparently they are one of the highest paid agencies in St Louis County. Talk about a pain to stay on top of everything.

Would like to compile a list of classes available to Paramedic/ EMT's for continuing education purposes. So here's where I need your help. Please add any classes that you know of. Please post the full name of the course as well. I was wondering of all the classes available, which would you say are worth it. Here is my list to date, some I plan on taking but others are FYI. Some have been left out like PALS and PEPP, ACLS as most of us know them well. ACLS-EP Advanced Cardiac Life Support-Experienced Provider EP-C- Emergency Pediatric Care ABLS- Advanced Burn Life Support AHMLS- Advanced HazMat Life Support FRT- Farm Rescue Technician AMLS- Advanced Medical Life Support BDLS- Basic Disaster Life Support ATLS- Advanced Trauma Life Support (Audit) STABLE-Sugar, Temperature, Airway, Blood pressure, Lab work, and Emotional support. (Neonatal) OBSTAT- High Risk OB transport CCEMT-P- Critical Care Emergency Medical Transport Program-UMBC (I STAND CORRECTED) FP-C- Flight Paramedic Certification GEMS- Geriatric EMS ITLS- International Trauma Life Support (Formerly known as BTLS) NRP- Neonatal Resuscitation EVOC- Emergency Vehicle Operators Course OK so what else is available out there in the world of EMS.

So back in 2004 when this was still under development, I read the proposal and from what me and others interpreted was this: In 2010 when this in implemented "All" paramedics national or state certified will be classified as "Advanced Emergency Medical Responder" with a few skills taken away. With more education those designated as such can obtain their Paramedic certification and get the skills "BACK" we all have been practicing all these years, and no additional "new" or more advanced skills, just the same old skills. Now PLEASE hear me out this was 4 years ago when this was all being drawn up and have yet to look over the NHTSA guidelines, so this may have all settled down a bit. I just don't see how making people whom have been Paramedics for years be reclassified and taking skills away unless they get more education to get their "Paramedic" title back. I can see a lot of Paramedics tapping out of the EMS business to do other things. Unless they are going to pay more for paramedics than the current pay rate. Excelsior RN program is looking more interesting to me now. Could someone who knows what is going on or someone who has the time to go read the 200 page guidelines from the NHTSA and let us all know WTF. :?: Take care.
