Have you thought about becoming a tactical medic? Some sherriff depts have them, it may fulfull your need for both.

Yeah as I hear most institutions do not want any tattoos of any kind showing, but I have seen several paramedics with a lot of tats and they were very professional, so it has to do with who you work for to whether or not they're willing to look the other way.
