I agree with AZCEP. It is actually an isomer of the compound levorphanol. (same molecular formula but a different structural formula) Levophanol is an analog of codeine. When talking about it's opiate like effects, one must realize that several opiate receptors exist, and the action of this medication only involves specific receptors. So, no analgesia with Dextromethorphan (DM). As I understand, Narcan may or may not help with OD.

In addition, the active metabolite of DM dextrorphan acts very similar to PCP and produces the behavior and neurological effects we associate with DM OD. I also understand that DM is a serotonin agonist and reports of serotonin syndrome have been associated with people taking DM and antidepressants.
