    I'm glad to see you update, Tyler had to link me here. I've been wondering about you lately. I've missed you.

  2. When I come on shift, the entire ambulance gets cleaned. Seats, buttons, steering wheel, door handles, all patient care equipment, floors are swept then mopped with a water/bleach/some kind of cleaner mix.

    Some people at my service don't seem to think that because the patient never really "touched" the stretcher (on bed sheets from the sheet lift, or our blue sheet) or if they are not on isolation percautions or leaking fluids that we don't need to clean everything... however, many of my patients ARE on isolation percautions or immunocompromised and I'd rather not play russian roulette with my patient's (or mine either!) health and wellbeing, so I/my partner wipe down all used patient care equipment (stethescope, BP cuff, change the pillow case, pulse ox, monitor leads, stretcher including the stretcher handles and up and down buttons, etc).

    We use whatever my company provides us with. Usually sani wipes but lately we've been getting some kind of thick foamy wipes with bleach that I like a lot better than the cancer wipes.

  3. Dust forwarded this video. IT IS SHEER GENIUS. What a great way to vent frustration and identify an issue. I have told everybody to boycott the trauma show and A.J at JEMS is doing the same. I have emailed the link to everybody I know and they love it. I could not resist the chance to send the link to Randolph Mantooth. He'll laugh his ass off. I'll let you know what he says (he loves this kind of stuff). Again, sheer genius lurks somewhere in the confines of this list.

    Boycott? But it's just such a REAL depicition of EMS!!! (That was sarcasm, love.) It's so much fun to make fun of it, but I don't think boycotting it will do much, it seems destined to crash and burn on it's own. Hopefully it's DNR is valid and properly filled out.

    Stress Cardiomyopathy Can Occur with Routine Procedures

    BALTIMORE, March 25 -- Stress cardiomyopathy, the so-called broken heart syndrome, can be precipitated by even routine procedures that involve catecholamines and beta-receptor agonists, researchers found.

    Nine patients showed immediate onset of the syndrome after intravenous administration of standard doses of dobutamine (Dobutrex) for stress echocardiography and epinephrine (Adrenalin, EpiPen) under a variety of circumstances, Ilan S. Wittstein, M.D., of Johns Hopkins University, and colleagues reported online in the Journal of the American College of Cardiology.

    The findings from this case series provide evidence that the effect of sudden, extreme emotional or physical stress on the heart is causally mediated by excessive sympathetic stimulation, the researchers said.

    The syndrome causes profound, but reversible cardiac dysfunction that feels and looks much like a heart attack.

    The characteristic clinical features include mild cardiac enzyme elevation in the absence of obstructive coronary disease, a prolonged QTc interval and diffuse T-wave abnormalities on ECG, ventricular ballooning, and rapid recovery of LV systolic function.

    Since the syndrome was first described in Japan in 1991 -- where it was given the name takotsubo cardiomyopathy for the resemblance of the characteristic apical ballooning to a pot used to catch octopus -- reports have linked it to unexpected news of a death, armed robbery, and even a surprise party.

  6. Would you say that community colleges generally have good programs?

    Valencia has the BEST Paramedic program in the area. The program surpasses hourly requirements, requires a&p prior to taking any paramedic class and hold their students to strict attendance and grade standards. SCC is a good school too, but if you talk to the graduates from either you can tell the students from valencia more often have a better grasp of the course material. That's just my personal observation. Sadly I didn't go to Valencia.

    Your golden ticket to nursing would be getting a job at Florida Hospital, they have tuition assitance after you've worked for them for a year and if you make the dedication to work for them for so many years after you finish. However, I think you have to maintain a certain GPA. Florida Hospital does have their IFT ambulance service, but it's really hard to get on to. But they hire EMTs for tech positions, etc.

    Outside of FHEMS, you have Health Central EMS, Rural Metro EMS, American Ambulance, EVAC, and Lake/Sumter. Rural Metro is supposbly losing their only 911 contract this year, and will probably fold after that, I suggest crossing that off of your places to apply after school list. Health Central is extremely hard to get on to. American Ambulance is an easy hire but I've heard the company blows and the best part about it is the ability to relocate to their Miami branch, I've heard good things about EVAC and I know two people who just got hired at Lake/Sumter EMS.

  7. To use some City veterans as examples, I've seen Dustdevil characterized as a cranky, BLS hating, Canadian disrespecting, arrogantly mean-spirited dude.

    That is the persona he likes to portray, but he's really a big soft teddybear.

    But, OP, you gotta roll with it. Don't let other people's adverse criticism of others bother you. The ones who do the criticizing are usually buttheads anyway who feel threatened by someone more intelligent or better spoken than they are or don't take criticism well. Not worth your anger.

    Glad you are still around to tell that story ak. Must have been pretty scary.

  8. I'm conflicted on this issue, truly. I usually advocate illegal immigrants, but that is a LONG time for a patient to be in the hospital. If alternatives were offered to the family and they kept rejecting them, what other choice did they have? They can't go along providing free treatment while the patient's family refuses to accept any alternative placement.

    However if there was no attempt to find alternative placement then they are truly in the wrong.

    At any rate, I hope that they lose the lawsuit because I don't think the patient or his caretaker will see a penny of that money either way.

With what was informed to us at this conference, the level of 110 does not significantly increase the risk of bleeding out much more than 90, still adequately perfuses the brain, producing not only an alive patient, but a patient with the greatest chance of recovering function and not having injury due to ischemia.

    But perfuses it with what? Koolaid?

  10. My spouse doesn't like this arrangement and thinks that it is an opportunity for relationships to form or someone to make an accusation of some kind of harassment or assault.

    It sounds like your significant other has trust issues. But before I go running off into an eye rolling, scoffing rant against your spouse I have to wonder.

    Do you do anything to make her worried about the possiblity of you wandering? Do you stare at other girls, make comments, make her feel like you're interested in someone else?

    If not, she needs to get over it. She's not your mommy and she doesn't get to decide who you do and do not work with. In tough economic times she should be grateful you have a job. To decline that assignment based on your partner may at the very least make you look difficult in the eyes of your employer, and at the very most, could get you fired.

    I never understood women who didn't feel their partners were capable of a platonic relationship with a member of the opposite sex. I'd find that mentality insulting if I were a male.

    Just as a side note, as a female, I always took care not to do anytihng that could be taken as flirting with my male partners. Work is work, not somewhere to pick up a chick or a dude.

  14. The bus driver wrote on a misbehavior form that a 15-year-old teen passing gas on the bus on March 16 to make the other children laugh, creating a stench so bad that it was difficult to breathe. The bus driver handed the teen the suspension form the next day.

    It wasn't as if he had an abnormal amount of gas, he was farting to make people laugh and be obnoxious. I don't see a problem in the suspension. It falls under "disruptive behavior"

    Are you not the one who just said a few posts back that this is a black and white issue and there is no middle ground? So there IS a middle ground only when it fits into your arguement?

    A woman and a man both have the right to choose what happens to their own body.

    How many of you have sex here just to procreate? Sex is a part of any normal and healthy relationship, but being in a relationship does not mean you and your partner are ready for the responsibility and raising a child. Does that mean you should deny your very human, and very real urges?

    We all know birth control and condoms are not 100% perfect and both have a failure rate. Very slim, but a failure rate indeed.

    What happens to these unwanted babies after they're born? Another baby for the adoption limbo? Or another mouth to feed when the mother is having trouble just feeding herself and ends up as another welfare baby? Doesn't matter, right? People just care about forcing the mother to carry the kid.

    I'm pro-choice. I would never have an abortion, but I'm not going to deny people who don't want a baby the right to abort that baby in the early stages of gestation.

    ing that it was emotional trauma? Notice how it's "Her daughter" and not "The baby she was aborting"? She has no right to money.

  16. What constitues fat? BMI?

    If an "obese" person can perform their job requirements as good or better than their skinnier counterparts, then they should be hired. If they can't, they shouldn't. This isn't a hard concept.

    "Weak" "Lazy" "Incompetent" and "Unable" are not synonymous with "Obese".

  17. I have been denied employment due to my weight because I am a large guy. There are certain flight programs where I exceed the minimum standard set for flight ready weight. Should I be exempted because I am a decent flight medic? No, I accept the terms they lay out and I either meet them or forget about that position. I am not entitled to that job simply because I want it.

    That's different, there is logic behind that weight restriction, but generally ambulances are not as concerned with balancing weight as helicopters are. The logic behind weight restriction in ground EMS is "They wont be able to do their job!" which is BS. I've seen skinny as a rail people who are unable to perform the essential functions of their job, i.e carrying a patient downstairs or across rough terrain. And I've also seen some "Big boys" and "big girls" who were more able to lift and carry and get on the ground and intubate than their skinnier counterparts.

    Instead of enacting a weight limit on ground EMS, a comprehensive and universal skills test would be better

  18. This is a ridiculous idea.

    One's weight has little to nothing to do with a person's ability to perform their job.

    I'm all for testing of one's abilities to meet the demand of their job but a "weight limit" is ignorant. BMI doesn't account for muscle mass or body type, just that if they are this tall they should weigh between this much and that much. By BMI index a lot of body builders or muscular men and women would be considered obese.

    One's weight does not necessarily mean they can't perform essential functions of the job such as having the ability to intubate someone on the ground. Not every woman carries the weight at her breasts, some gain weight around their hips.

    Instead of setting a "weight limit" let's have a universal test that screens for the ability to perform the functions of the job.

  19. Compressions only CPR works under the theory that there's enough oxygen bound to hemoglobin and such to circulate while waiting for an ambulance to arrive. Every time you interrupt compressions to deliver breathes, you're interrupting circulation to some vital organs since you ARE the circulation. I think this also works on the theory that you'll get an ambulance pretty quick... I doubt this "new CPR" was made with rural and very busy areas in mind where you may wait for 10+ minutes for an ambulance to arrive.

  20. In my lovely little Orlando area, there are a few companies that still do 911.

    Rural Metro still runs some 911 calls. Some. But they're also about to die in central florida and pull out. American is kicking their butt in IFT calls.

    Health Central is a hospital based EMS 911 response, it covers some theme parks plus some of Ocoee. They're actually currently hiring for EMTs (I want that job! Arrgh!)

    Reedy Creek EMS is in Disney, which is kinda "911 response" and you get the little golf cart ambulances. Who wouldn't want that?

    EVAC is in Daytona Beach, they still do 911 response (last I heard. But I also heard they suck as much as Rural Metro does.)

    Lake Sumter EMS

    Polk County EMS (Actually requires you to be a resident in Polk County.)

    Lee County EMS.

    Orange County FD is hiring NON FD Medics for roaming Medic trucks. 12 hour shifts. They do require you to become FD within a certain time, but they pay for it. You must also pass the same fitness test their FF do.

    There is still SOME job ops for Non Fire EMS out there, but they don't pay very well and they're kind of hard to secure.
