  1. And to think, I miss all this inflammatory crap. Wow! A question turned into such drama.

    I’m not going to discount what anyone has offered, however, I am just saying what I would do and what I was taught.

    In this exact situation, I would immediately pace. I’m sure IV access on someone in this circulatory compromised state, would be difficult. I’m not about to waste time looking when I can pace within 30 seconds.

    On the topic of atropine itself, I was taught that is was actually contraindicated in high degree heart blocks, as it could cause significantly worsened rebound bradycardia. But I guess if the pacer is there, it doesn’t matter huh? I’d have to go back to my 10 year old pharmacology book to see if it’s actually listed as contraindicated.

  2. Hugely inappropriate. If the mother agreed to this, then she's trash and probably provides minimal parental guidance if any.

    If the nurse actually did this, she a piece of shit and needs to be fired and have her license pulled. If someone even THOUGHT about doing that to one my children, I'd probably be arrested for punching someone in the throat. That is absolutely absurd and I pray to God that bits and pieces of this story are even not true, or embellished.

  3. Tammy Tisa, 55, of Cumberland County New Jersey was laid to rest August 11, 2010 after succumbing to cancer. The news of her death was a terrible shock, as she passed away only 8 days after being diagnosed with breast cancer.

    Tammy was Assistant Chief of Lawrence Township EMS (Station 38) and has also served as President and Treasurer. She was also the Director of Training and Assistant Administrator for the Millville Rescue Squad.

    Now, on to my rant. I havent posted on here for well over 3 years, but after reading a news article on Tammys passing with pictures (www.1strespondernews.com) I became, I guess enraged is the word.

    Millville Rescue Squad should be ashamed of themselves. Here was the sweetest women, nicknamed Mamma T for a reason, who gave everything to Emergency Medical Services. You paid tribute to her, by allowing your squad members wear short sleeve shirts and white gloves. Most of your members couldnt even tuck their shirts. Then my rage became worse when I saw the "Honor Guard" WITH SUNGLASSES while at attention! Next, the Chief, of all people, presented the flag that was draping her casket to her surviving husband, while wearing his sunglasses. Even the people carrying her casket were wearing sunglasses and short sleeve shirts.

    You people are a joke and you wonder why the joke of Cumberland County is Millville Rescue Squad. I cannot fathom the amount of disrespect you just showed for Mike and his family. You cant tell me for one second that you couldnt scrap together some long sleeve shirts at least for the people involved in the procession. I am so angry about this I cant even stand it!!!

    When I attended funerals for police officers, EMTs, or firefighters, as a stand-by medic unit, not even involved in the funeral itself, I ALWAYS wore a pressed long sleeve shirt and a black tie to show my respect. You people make me sick and Tammy deserved so much better! I hope you are all proud of yourselves for this blatant display of disrespect.

    Oh, and I forgot one. One picture shows a women at “attention” with her pager on. Why? Were you going to respond to a call during the funeral?? I’m pretty sure Vineland EMS was probably called in to cover the squad so that you guys could attend the funeral. Again, goes to the disrespect and lack of professionalism.

  4. If every other State can do it, why can't they? Well, because of the mental midgets at the First Grade Council. Opps. Sorry, read First Aid Council, my bad. They believe it's perfectly acceptable in this day and age to have volunteers respond who have nothing more than CPR training, ambulances that do not require ANY minimum amount of ANY equipment, and any response time is perfectly acceptable. These numb nuts need to go first. Then maybe we'd make some progress.

  5. That is in fact a HIPPA violation and they had every right to fire him. The ONLY acceptable time when you can photograph a pt, is on the street. There is no expectation of privacy. You CANNOT take any pictures while inside their home or inside the ambulance where they have an expectation of privacy.

    I've worked with paramedics that I would trust my 3 year old with. I've also worked with medics whom I would not let near my dog.

    I've worked with nurses..........same as above.

    Stop bashing each others profession. Each has different requirements, educations, and personalities. Neither is any better than the other, education or not. Like I said, I've worked with nurses who have a masters, and I was better off talking to a box of hair. And likewise, I've worked with medics with umteen hours of education and experience, and the same thing applies.

  8. It seems strange to me that this discussion is even happening on EMT city. As health care providers, it is imperative that we maintain a neutral attitude towards alternative lifestyles when we are doing our job in order to provide the best care to everyone. And the same goes with firefighting. When someone spends time considering the negative actions/history of a group of people (groups that are seemingly created by skin color), and create judgments on those groups, it can easily cast a cloud on your work ethic when dealing with those groups that you think you have a problem with, or likewise have a problem with you. And it is easy to say and believe that you will not pass judgment when that time comes, but I think that the subconscious is a very powerful thing, and if you commit yourself to believing that these boundaries exist by paying more attention to the things that we believe make us different, instead of the things that make us the same, we will continue down a path of biasness, which will affect our actions and thoughts. Forgive me if this is to radical to get into... I just don't care for racism.

    And I can honestly say that in the past, have have in fact seen the impact of racism (both towards me and radiating from me), it is not productive. There is no reason to let your ego get jealous just because someone uses a word that we were told not to use.

    It's not at all strange. You made a good argument though. Very thought out. However, this is what people do. Not to mention people in this field. We talk. My life does not revolve around 100% EMS or fire. I want to discuss over topics. Whether or not this is the place to do it, remains to be seen. I post here because I know the majority of the frequent posters. I realize they can hold discussions outside of fire and EMS.

    No it does not impact my care on the street, whether you think so or not. I treat the pt according to their complaint, not their skin color. I realize that not every black person agrees with Sharpton, I realize not every person from the Middle East is a terrorist. Would I treat Sharton differently? Probably not. And only because I know he he has good lawyers. :P Otherwise, well, nevermind. But outside of that individual, someones religious beliefs, sexual orientation, skin color, creed, or otherwise, has never been a basis for my treatment.

  10. The one thing that bothers me is that these two Reverends haven't been exactly preaching. When was that last time you heard either one of them say, "JESUS is the answer to all our problems"? I don't believe that I have, ever. They only "talk" about Civil Rights and social problems. When was the last time either one actually preached a sermon on salvation?

    I'll stop for now.

    I here ya. Everything they "preach" is about how blacks are oppressed by the whites, but never any references to God or Jesus, or even Allah for that matter. But what gets me is how they went after Imus, but it's all good for Jackson because he apologized!! I don't get why people still support thesetwo ignorant morons!!!

    I'm really at a loss here. These two peach about bringing out the best in the black race and yet, they drop the "N" bomb. Both of them have been guilty of this. WTF???? It is so frustrating that what's good for them, isn't good for us. I can guarentee you if a white person said it, they'd be fired!

    Do you know how much they pay an hour. I am interested in becoming a flight paramedic. I also see that they have another position of paramedic is that in a prehospital enviornment or is that in a hospital.

    It sucks big time. I think Air Methods (currently runs LifeNet in Tallahassee) was something like $14 an hour for a medic. And they work 24 hour shifts. I don't know what copters do Volusia, but it will probably run about the same. EMS here sucks too. As an EMT, you're lucky to start NEAR $30K and as a medic maybe $35-40 if you're lucky.

    Go work for the feds in that area. Coleman (corrections) hires medics and sometimes EMTs for 45 a year. And its a cake job. Just thought I'd throw that in there.

  13. I had a little old lady fall in the parking lot of a well established food store in NJ. She tripped over those concrete bumpers that stops the cars from banging into each other. She was screwed up, broken hip, wrist, and ankle. That supermarket paid for everything. Then she tried to sue for millions saying she tripped in a pothole. When her lawyer contacted me I royally pissed him off. Right on the front page of my report, it says "Chief Complaint" with three little lines. There were also check boxes. Anyway, I was told from school, that you wrote EXACTLY what the pt says is wrong on those lines. Then you also check off the appropriate check boxes for nature. She stated and I wrote "I tripped over the parking bump thingy". He was pissed when I refused to offer a different explanation. I told him that was exactly what she said to me, and it got thrown out of court. And you would think that company would have given me a $100 gift card or something since I saved them millions.

    And now, I got supoened the other day for August. It was for a stabbing. We were o/s for 3-4 minutes and I drove. FD did most things before we got there, so we scooped and ran. I drove, so I don't have much to offer. I'm waiting to see if the lawyer will let me off.

  14. I like to play "Stayin' Alive" during a code. Or "another one bites the dust". It's a good beat for compressions.

    I also like to play Ozzie over the radio when the volunteers go to a wreck and everyone's screaming on the radio. Ozzie : "Voices. Voices. A thousand thousand voices. Whispering, the time has passed for choices." That's my favorite one! I love doing that. Especially before they put the identifiers in the radio. Although the dispatchers always knew it was me anyway.

  15. Say your service carries morphine 10 mg in 1 ml of saline. Is there a benefit to say drawing up an additional 10 ml of saline so you have 10 mg in 11ml etc.? Some have told me that this makes it easier to push or smoother for the patient. Any truth to this?

    Draw up 9 cc's NSS into a 10 cc syringe. Add your 1 mL of MS. This makes the concentration 1 mg/mL. Easy to view and easier to push slowly. Does nothing for the comfort of the pt. Just ease of figuring out the dose. And if you use carpujets, just revove the end that you normally would connect to the pt. It can be drawn out into the 10cc syringe this way.

    Anyway, having come from that system to Florida's :D I can argue some.

    That it is hospital based, does not necessarily make it better in terms of QA. Our medical director here is very much involved. (County based and employed, well PRN anyway. Now I'm a federal slob..haha). He stays abreast of new technologies and is all for us using it, if he feels it's necessary.

    The system in NJ needs to scraped and started brand new from day one. They need to just start over. Regionalization is the only way to help. Working up there, lets see, every municipality has it's own 911 system. They answer the call. Those dispatchers would call if they felt a medic unit was necessary, which most of the time it was not but whatever. Then we were dispatched. Then we had to figure out what channel they operate on. Was it on the 800 mHz system, which they only allowed the medics to access a few years AFTER the squads were on it. Or was it the VHF repeater. Or was it, guess and hope someone on the ambulance would answer you. Every time I responded to a cluster MVC with multiple agencies and vollies bagocking on the radio, I always played the same song over the radio system. Maybe you'll recognize it. "Voices. Voices. A thousand thousand voices." So let's start with that. Regionalized dispacth for each and every county. For the smaller counties, maybe they'll combine and work as one.

    Next, EMS needs to regionalized. Every county (same thing - small ones maybe two) needs to have regionalized units. I've grown kind of fond of the system here. It's an EMT and a medic, sometimes two medics depending on staffing, but never just two EMTs. Maybe throw in a chase vehicle or two for added support during traumas or something. Down here, the fire departments are sometimes ALS so we can grab a fire medic if need be. The thing I don't like about this system, is that your skills diminish. Otherwise, every single truck is ALS with at least one medic. EVERY patient gets a medic. So that unknown medical call that is now a code, doesn't have to wait an additional 15 minutes for an ALS unit. (Yeah, cause I've never been dispatched to that... :D ) Every thing would be regionalized. EVERY city/town pays for the service equally (oh well to those smaller communities - deal with it). Or I guess I play fair, maybe it should per cap. Whatever.

    Next, going back to skills diminishing. EVERY single employee is retested every 6 months (maybe yearly depending on call volume.) This includes EMT's. Give scenarios suited to their licensure. EMT's do BLS, medics obviously do BOTH. If you can't pass, bye bye. This needs to be overseen by DOH so medical directors can't circumvent anything. Make the test come from DOH if needed or something like that. Every medic needs to successful tube XYZ amount of people in 6 months. If need, you have to go do 8 hours in the OR and tube some people (preferably a trauma center or something so it's realistic to the street.)

    The two tiered system needs to go and regionalization is it. It's the only fix. It's not fair to the EMS and it's definately not fair to patients. I could tell stories for days about people waiting for an ALS unit with someone acutely ill, and they suffered. That's another discussion, but still. It needs to stop. And the only way it will stop, is regionalization. The region is responsible for EVERYTHING. Transports, 911 transports, interfacilty, back to nursing homes (my favorite passtime), air transport, EVERYTHING.

    Even better! Three regions, three employers and that's it! South, central and north. Each one covers the entire area from BLS transports to ALS to air. That way, the little bumscrew towns are covered with the same staff working in the cities. Make it state operated so it is in the state retirement system. There goes that complaint of no retirement.
