ERDoc
Your UA is neg. Flashlight is normal. He says his immuniazations are up to date. Your CT with IV contrast shows nothing intrabdomenally, but there is some mild stranding around both testes.
chbare
Any soft tissue gas noted or suspected? Prehn sign? Can we do an US and try to rule out torsion? Take care, chbare
EMS-Cat
Don't know if this has already come up - my web reader is on the fritz Did he, by change, take any ' nitrates [Viagra like meds]? Could he be in that 1% or so that gets testicular pain with them as a side effect? Has he had any shift in color vision [blue shift]? Also, in consultation with my SO [who's also of a medical background], he asked for an abd US to r/o a dissecting AAA [he did a lot of his EMS in a farming area and said this was one of their zebras: young, healthy male doing a lot of heavy lifting who developed slow onset bilat testicular pain] Like I said, zebras, but things come to mind... Richard B the EMT
Your UA is neg. Flashlight is normal. (From a BLS) What is meant by the flashlight, in this usage? Eyes what I would call PERL, or Pupils Equally Reactive to Light?

No meds. No blue vision. CT was neg, so you don't need an US (it isn't very sensitive for disection anyway).
ERDoc
(From a BLS) What is meant by the flashlight, in this usage? Eyes what I would call PERL, or Pupils Equally Reactive to Light? Someone had asked to transiluminate the testicles with a flashlight, which was negative. So, you have an otherwise healthy immigrant with fairly sudden onset of fever, testicular pain and TMJ pain. This dx is a clinical one and is confirmed with labs. Anyone have any guesses?
EMS-Cat
Sorry ... missed the contrast CT mentioned earlier. Could we have the lab results, please?
