Locked?

Do you have a lot of lidocaine and dopamine abuse in New York? :?

And doesn't that make it kinda hard to start a drip in the house?

When the state (NY) decides to go out and inspect units, the inspector tends to have one particular "pet peeve" which changes from time to time. Right now, they're looking for meds to be in locked cabinets on the ambulance. I guess a junkie might want to steal my narcan when I'm not looking.

When working inter-facility:

Adenosine 6 mg / 12 mg vials: 30 mg total

Albuterol x 4

Amiodarone 150 mg x ?

Atropine 1 mg bristol x 6

ASA 81 mg orange flavored chewables x 1 bottle

Calcium Chloride bristol x ?

Dexamethasone vial x 2

Dextrose 50% 25 grams bristol x 2

Dextrose 25% bristol x 1

Dextrose 10% ampule x 1

Diphenhydramine 50 mg vial x 2

Dopamine 400mg vial x 2

Epinephrine 1:10,000 1 mg bristol x 6

Epinephrine 1:1,000 1 mg ampule x 3

Furosemide 100 mg ampule x 2

Glucagon 1 mg vial set (powder/sterile water) x 1

Lidocaine HCL 100 mg bristol x 4?

Magnesium Sulfate 1 gram vial x 2

Narcan 2 mg bristol x 5

NTG Spray Bottle x 1

NTG Paste tube and measuring papers x 1

Oxytocin 10 units - I think we're out of this temporarily

Vasopressin vial x 2

Sodium Bicarb 100 mEq bristol x 2

Thiamine 100 mg vial x 2

Tetracaine solution single use tube x 1

Various size Normal Saline Bags and Pre-filled Syringes.

1 50 or 100 cc bag of D5W (only for use with Amiodarone)

Separate Sealed Kit:

Diazepam 10 mg tubex x 2

Etomidate 20 mg ampule x 2

Morphine Sulfate 10 mg tubex x 2

At my 911 job, our portable bag has roughly the same as above. We also keep a significant overload in a separate box to restock out of.

Also carry:

Cardizem pre-filled syringe

NTG Tabs (instead of spray)

Metaproterenol

Solu-Medrol (maybe)

1 big Proparacaine bottle (instead of tetracaine)

Narcs there:

Versed, Valium (50 mg honeypot), Morphine, Etomidate

we have about the same things in our drug box. all the usual stuff. what i would like to do is have a smaller more portable drug box of just the bare essentials for the faster paced calls like full arrests and anaphylaxis.

does anyone else have a separate drug box or bag for that? if so, what's in it?

I sorta do. I carry all my injectable narcs on my belt, and IV antibiotics in the trauma bag. Trauma is the most common "fast paced call" out here, so that works out well. With those two things on me at all times, I rarely ever have a need for my drug bag.

someone actually still has romazicon on the truck? Dangerous drug and was pulled from most protocols....suprised to see it. As far as us, lots of drugs, but we use the little like tackle boxes with the compartments. Fit in the bag like a glove and seriously reduce the bulk. We love it.

Since I last posted in this topic, protocols have been updated. I now carry the following in addition to the last list...

Reglan

Metoprolol

Amiodarone

Haldol

Acetaminophen elixir

Benadryl elixir and tabs

Solumedrol

Thiamine

I would have liked to have seen a few drugs on their, but what do I know?

Incidentally, does anyone have experience with Vasopressin prehospitally? Any interesting articles out there? Think I've found one, but I'm really interested in it's use in place of or as an assist to epi in cardiac arrest.
