BEorP
I don't have the full interpretation with me at the moment, but there was no mention of the possible electrical alternans and the overall interpretation was AF.
mikeymedic1984
It's not afib -- consider typical SVT or junctional tach. The rhythm is not irregularly irregular.
scubanurse
Uhh definitely irregular.... Did you actually measure this out or just eyeball it?!?! If you get out the calipers or even a sheet of paper you will see that the r to r interval is definitely not regular....
chbare
The last time I said this I was tore up a bit, but it looks like atrial fibrillation to me.
Job13_5
Being bold, I'd not call it AF because it appears regularly irregular. The R-Rs appear consistent for every other beat...ie two different foci working as the pacemaker. So it wouldn't be electrical alternans either. Edit: I should add the caveat that I'm only using a piece of paper to mark the R-Rs from the pic in the phone (no calipers)...but it appears to march out consistently on the running strip. Hal Lineback
Just a guess! Irregular, accelerated jucctional?
BEorP
Just to give a little no-update-update, I haven't been able to come up with any reasoning for the variations in QRS amplitudes. It might end up going unexplained, but if I ever figure it out I will update the thread.
