  1. I believe it was touched on in a earlier post, but confined space heaters are portable and actually work quite well in these conditions. You can take the propane heater a bit away and take the attached tube and run it right into the vehicle to directly heat the PTs and rescuers. Nice thing is you probably won't have to spend a dime to get one. It is indeed a looming problem, it's hard to justify a huge percentage of a fire department's budget to pay for people that don't work there anymore. Sadly in a great many cases this stems from the cities not properly funding their pensions in the first place, or borrowing from the fund hoping that tomorrow would never come.   5. Ok, here is a useless drug, before we had Albuterol we had...... Trebutaline!!! Give it sub Q and wait for nothing to happen. And who remembers High dose Epi? My highest score was 38mg on a cardiac arrest.
  6. Well honestly if you get a normal 2WD you will have the same wheel base and tires, however you will probably have a high stand over and thus a taller truck being more top heavy. We have been running ford F350/450/550 for years, but all 2WD, never had problems getting stuck.
  7. Watch out getting a 4X4 ambulance, the ford fronts have poor turning radius with a live front axle. Sadly Chevy had a great 3500 4X4 front end but for some reason they stopped making it. Also if you put on allot of mileage your going to have problems with a 4X4.
  8. Funny thing about the Litmans, we are having more war veterans join our ranks at the FD, and many of them have significant hearing loss due to getting blown up frequently. We have bought a number of the mater cardiology IIs and some of the more expensive electronic ones, but the verdict on those is still out.

  10. We actually have a High School EMS program, when you graduate you can take the national registry after your 18th birthday. Great success with it and had allot of kids go on to get full time jobs. Plus like Mari said, it's great to have some young blood infused into our departments.
  11. Your NR should be able to get you hooked up with allot of places, but it's the short term employment thing most people are going to have a problem with. Are you looking to get hired or just do ride alongs? We have people from all over the world ride with us from time to time, but not to offer employment.
  12. When I was in medic school I always studied at the local coffee shop, nice and quiet, ready access to caffeine (before the days of wifi even). One day I was banging my head against my books on the table trying to make the cardiology go inside when a older gentleman walked by me and said "Trying to learn cardiology huh?" He was the head of the Heart center at the hospital, and I learned more from him in he 20 minutes he spent talking to some dumb medic student while waiting for his wife, than I did my entire year of Cardiology in school.

  14. Surprisingly enough money isn't an issue and people do get paid $15 per hour per run, and we are not a transporting service. I work full time at a ALS transporting department and spend enough time trying to write reports for billing and cost recovery I don't think that would fly well. Although you do raise a good point, as an officer I get paid by the department monthly, the rank and file gets paid quarterly. I do think money is a motivator and I also believe it should be a issue that gets looked at.

  16. Well we are a modest Fire/EMS department, although we had a record year this year of 160 runs! Up 30 runs from last year, We have around 2000 people in a mostly farming and freelance pharmaceutical production. Our department is around 7 people strong a his time, mostly kids under 25 in college.

  18. Treat her as a trauma PT with an eye to her possible cardiac symptoms. I personally wouldn't give NTG or ASA due to the possible chest trauma, but would consider Morphine if her LOC is good. I would also focus the assessment on possible chest trauma, LS, physical exam, and 12 lead. Then work out cardiac as much as I could.
  19. Are you using this "box" Method because that is what will be tested, or because you want to try it? I would just keep it as simple and uncomplicated as possible. If you have access to your skills testing sheets go through them over and over, and then have someone read it off to you while you are doing your practice. No field tricks needed or wanted here, just follow whats in the book because that is the standard you will be tested against.
  20. I had a employer that refused to pay us from 0230 to 0630 everyday unless we were on a run, he gambled that we would stay in our bunks instead of going home if were weren't getting paid. I'm guessing this is probably the same scenario. (yay private service)
  21. You say this as ignorant idiot. To say that all fire services provide an inferior service than for profit EMS simply means your opinion is as useless as you seem to be. I have worked for Public and private EMS, hospital based and in the ER, and the fire service I work for now has the greatest dedication to the quality of care we provide than any of the other places I have worked. I know of fire departments that do not have such dedication and I know of private EMS services that are only out there to make a buck. If you can't be a fire fighter that is dedicated to providing the highest care
  22. I have actually seen a few students have to redo their internships because of poor performance. If they do not mention it in the syllabus that leads to a problems because they are subjectively graded. If it were my class I would have you moved to a different agency and placed under a different Paramedic FTO.

Intubation is still the gold standard for securing an airway, blind insertion airways have been gaining popularity, and every year I hear that paramedics will no longer perform ETs, But I still see all the air medical services(around here anyway) using only ET tubes, as well as in the ER's. Feel free to correct me if I am wrong, and I would like to know of services that no longer use ETs as well!
