  1. This is one of those kind of things where you wish you could say something smart back to the pt...Well, it wasn't me, but the medic who came on scene and was familiar with this particular pt. Dispatched to a residence class 1 (L&S with ALS dual dispatched) for a hemmorage. Was advised that etoh was present other than that no info. Came on scene and knocked on the door. No one answered. Finally, opened the door and motioned to wait a minute. Finally pt comes out and is obviously intoxicated, said she wanted us to come back tomorrow to take her to the hospital. Medic says "Lady, we aren't the gas people, we can't just come back tomorrow" the look on my partners face was priceless. :-) Edit- no hemmorage was present but it was a psych pt instead.

  3. Can and should are two very different things. Sure, CAN shock someone on a stretcher with no backboard. I've seen it done with the pt on a reeves and on the stretcher. A hard surface just makes compressions much more effective, not sure it effects defib. Just don't touch the frame.

  5. My Sup and I and my preceptor were having a discussion yesterday and he brought up Aortic Aneurysms due to traumatic injury. His thought was since having the pt in an upright position during transport is standard as laying the pt down would cause them to bleed and ultimatley go into arrest, would a KED be effective in to keep the legs up and then lay pt down on litter. Our thoughts (my preceptor and I) were that the muscular flexion when the pt is sitting up is the cause of the decreased blood loss, not the postion of the feet as if the pt was sitting. So if the pt was in a sitting position but laying on the litter, the muscles would relax and blood loss owuld resume. I was wondering if this is the case or maybe my Sup has a point.

  7. I was on a call and I'm new and precepting and the medic was telling me four things that can cause a pt not to have a pulse even when CPR is being done properly. Hypovolemia, pericardial tamponade, tension pnuemothorax and pulmonary embolism. I am pretty clear on why the first three would prevent a pulse but I was wondering how a pulmonary embolism would prevent a pulse from being present. I did a search and didn't find anything. Can anyone try to explain this?

  9. We were working a code and the medic called into the hospital to give report and she stated she gave the pt epi and atropine and he was intubated and cpr was in progress. We had an ETA of 5-7 minutes. She asked if there was anything else and the doc came back and said, "What are his vital signs?"
  10. It would seem to me that alot of companies would love to have a fresh medic as long as they are willing to learn and gather more knowledge even after completing school. I know as an EMT depending on the company, they want new EMT's because they get to mold the EMT or medic, into what THEY are looking for and you don't really have a set practice yet. Once you start, you get a routine and certain way of handling situations not to say that it can't be changed. The company I work for especially loves it because I'm in medic school so I can use knowledge not necessarily ALS skills but knowledge I learn in A&P and Pathology and such. Anyways, this got off topic but personally I am 20 and in medic school. I have always had an extreme interest in medicine to the extent of sitting and reading my medical dictionary. Instead of age limits we need maturity limits. I know here you ahve to go through interviews with the college's program director before being accepted into clinicals.

  13. As far as I know here, if anyone gets in the backseat of a LEO's car, they must be handcuffed, or so they say is the local dept's policy. As Richard said, not "under arrest" per say. Most transports here were done by PD when the hospital had a small psych area where they would temp. hold pts for a max of 24 hours I believe until they can determine if the pt is a harm to themslves or others. Now that the local hospital has no facility, I am not too sure. Only reason EMS would be called as was said earlier if the pt downed some pills or cut themselves already.

  15. No. Theres no state laws governing who can drive an emergency vehicle in Pennsylvania. Just the services and most require you to have first aid and cpr and hazmat operations and some form of Emergency Vehicle Driver Training. Also most companies have an age limit because of insurance reasons.
